Job Description

School to Work Alliance Program (SWAP) assists young adults ages 15-24 with mild to moderate needs in employment to become competitively employed and to achieve successful community outcomes. The SWAP Specialist provides these youth with pre-employment and employment services both in school and in the community. This program is operated through the Mountain BOCES and implemented in partnership with Division of Vocational Rehabilitation (DVR), local School Districts, Colorado Department of Education (CDE) and area businesses.
Qualifications Required Education:
Associate's Degree in related field or equivalent in years of experience in related field

Coordinator/Executive Director Contract Days:
200 (FTE=1.0), Beginning 8/1/2026 for FY26-27
Compensation:
Negotiated contract

Heavy lifting; some travel required
Beginning Salary:
Hiring Range $35,000-$60,000: Placement on hiring schedule depends on education & experience

Work and caseload will cover Lake County School District and surrounding area. The position is Year-Round and requires a flexible schedule so SWAP staff are available at various hours to provide direct services, both inside and outside of school hours (including during evenings, breaks and summers). This position will serve on site at both Lake County High School and Cloud City High School to offer pre-employment transition services, case management, and transition focused activities based on partner and student needs. This position will also serve as a liaison between schools, businesses and community organizations. This position will serve out of school youth up to the age of 25 who are seeking employment. Maintain an updated caseload roster tracking referrals, active participants and include contact information. Maintain additional documentation that supports record keeping and organization of services provided. Attend student IEP meetings to support transition planning as well as to build a bridge towards DVR services. Outreach in the community, support planning, and facilitate recruitment by attending Back to School Nights, College, Career, and Community Resource Fairs, and local business networking events. Participate in weekly SWAP Team Meetings as well as monthly
DVR/SWAP
Team meetings. Attend Mtn. BOCES Professional Development Days and LCSD school professional development days and staff meetings to stay informed of district policies and events. Support student internship opportunities and serve on committees relating to work-based learning coordination and ICAP efforts in schools.
