Job Description

Job Summary Nexus Family Caregivers is a community driven organization supporting people with intellectual and developmental disabilities to build connected, meaningful lives. We provide a variety of individualized services, including residential supports for people living with family caregivers or in their own homes and apartments, supported employment, community engagement, mentorship, and other services designed around each person's goals and needs. We are looking for someone who wants to be part of both providing great support and building great programs. This is a hybrid direct support and program coordination role. You will spend a meaningful amount of time working directly with the people we support, both one on one and in small groups, while also helping coordinate services, support staff, solve problems, plan programming, and strengthen our growing organization. As Nexus grows, we expect this position to take on increasing program coordination, supervision, and leadership responsibilities. Direct connection with clients will still remain part of the role. We believe the people leading our programs should know the people we serve and understand what services actually look like in the field. Responsibilities Provide individualized support in homes and community settings Work one on one with clients as well as facilitate small group activities Support independence, daily living skills, community involvement, relationships, employment goals, and self advocacy Plan meaningful activities based on each person's interests, preferences, and goals Help coordinate schedules, staffing, activities, and day to day program needs Build strong relationships with clients, families, staff, and other members of each person's team Participate in weekly team meetings and program planning Help onboard, train, mentor, and support Direct Support Professionals Provide guidance and leadership to staff working in the field Review documentation and help ensure services are consistent, person centered, and high quality Identify problems, communicate concerns, and take initiative to help find solutions Help develop systems, processes, activities, and programs as Nexus continues to grow Take on increasing responsibility for staff supervision and program coordination over time We are specifically looking for someone who is excited about becoming a leader. You do not need to walk in knowing everything, but you should enjoy helping other people succeed, be comfortable giving feedback, and be interested in supervising staff as the role grows. Schedule This is not a traditional Monday through Friday, 9 to 5 position. The people we support have lives outside of business hours, and many of the best opportunities for community involvement happen during evenings and weekends. This position includes a combination of daytime, evening, and weekend hours. At minimum, this role requires: Two evenings per month Two weekend days per month These are minimum expectations, not maximums. If you enjoy evenings or weekends, we can absolutely build more of those hours into your regular schedule. We also offer flexibility in how the workweek is structured. We are open to include four day weeks, five day weeks, combinations of both. Schedules will vary based on both staff and client needs, activities, meetings, administrative responsibilities, and program needs. If you prefer variety and flexibility over a rigid weekly schedule, you may really enjoy this role. Who We Are Looking For We are looking for someone who: Genuinely enjoys spending time with people with intellectual and developmental disabilities Is interested in both direct service and leadership Wants to help build and improve programs Takes initiative and notices what needs to be done Is dependable, organized, and follows through Communicates well with clients, families, staff, and coworkers Can work independently while also being part of a collaborative team Is comfortable solving problems and making decisions Enjoys mentoring and supporting other staff Is comfortable giving and receiving feedback Is excited about growing into staff supervision and increased program leadership Values choice, dignity, independence, inclusion, relationships, and person centered services Prior experience working with people with intellectual and developmental disabilities is strongly preferred. Experience with program coordination, staff leadership, supervision, case management, residential services, supported employment, behavioral health, or other human services is also highly valued. Requirements Valid driver's license and reliable transportation Ability to pass a criminal background check and motor vehicle record check Ability to read, write, and communicate effectively in English Ability to lift up to 30 to 50 pounds, with or without reasonable accommodation Ability to maintain a flexible schedule that includes some evenings and weekends Strong communication, organization, and problem solving skills CPR, First Aid, and QMAP medication administration training are provided and paid for as part of onboarding. Current certification is not required to apply.
