2996663117-2 Apply for this job now Job Description Hartford Healthcare is seeking a Wound Program Specialist to lead and coordinate evidence-based wound care services across our Healthcare & Medical Services department. In this role, you will assess and manage complex wounds, develop and implement treatment plans, and collaborate with interdisciplinary teams to improve patient outcomes. You will monitor clinical quality metrics, standardize wound care protocols, and provide staff education on best practices, documentation, and infection prevention. Responsibilities Assess and manage complex acute and chronic wounds using evidence-based protocols Develop, implement, and update standardized wound care plans and guidelines Collaborate with physicians, nurses, and allied health professionals on wound management Monitor and report wound-related quality indicators and clinical outcomes Educate and mentor clinical staff on wound care best practices and documentation standards Ensure compliance with infection prevention and regulatory requirements in wound care Serve as subject matter expert for wound products, dressings, and advanced therapies Participate in program development, process improvement, and policy updates for wound services Engage patients and families in wound care education and self-management Utilize EMR systems for accurate wound assessment, documentation, and tracking Required Skills Wound assessment and staging Advanced wound care therapies (negative pressure, specialty dressings) Infection prevention and control Pressure injury prevention and management Clinical documentation in EMR systems (e.g., Epic) Interdisciplinary care coordination Patient and staff education Quality improvement and outcomes tracking Knowledge of wound products and formulary management Regulatory and accreditation standards related to wound care
