PRIMARY FUNCTION
The Director, Patient Engagement - VBC Membership is responsible for leading outreach strategies and teams focused onbringing valuebased care (VBC) members into active engagementand recapturing disengaged or lost members. This includes onboarding newly attributed members, reengaging neverseen and disengaged patients, and driving member recapture through PCP alignment and reassignment strategies. This role requires deep expertise inpatientlevel and payorlevel population management, strong operational leadership, and the ability to translate attribution data into targeted, effective outreach campaigns.ESSENTIAL DUTIES AND RESPONSIBILITIES

VBC Membership Outreach Strategy & Execution Lead outbound outreach operations focused on: Onboarding newly attributed VBC members Engaging "neverseen" patients Reengaging disengaged or lapsed members Recapturing members through PCP reassignment and alignment strategies Ensure outreach efforts are personalized, compliant, and aligned with payor requirements and value-based objectives Population Segmentation & Targeting Oversee segmentation of VBC populations based on: Coverage status and attribution source Engagement history and care utilization patterns PCP assignment and visit history Partner with analytics teams to translate attribution and eligibility data into actionable outreach lists Prioritize populations based on impact, likelihood of engagement, and retention risk Member Recapture & PCP Alignment Own operational strategies to: Identify members who have not engaged with assigned PCPs Support PCP changes and reassignment processes, where appropriate Remove barriers to PCP establishment and retention Ensure outreach teams understand payor rules, attribution logic, and recapture opportunities People Leadership & Team Development Directly manage outreach managers and supervisors responsible for onboarding, retention, and reactivation efforts Build a high-performing team skilled in complex VBC member conversations Establish performance expectations and accountability for engagement and recapture outcomes Promote a patient-centered, empathetic outreach culture Performance Management & Reporting Own key metrics, including: VBC member onboarding completion Engagement rates for neverseen and disengaged populations Member retention and recapture rates PCP establishment and alignment outcomes Use performance data to drive staffing decisions, outreach approaches, and improvement actions Process Execution & Standardization Evaluate current workflows across outreach, scheduling, follow-up, and documentation to identify inefficiencies, risks, and opportunities Design and implement standardized processes, tools, and best practices to improve consistency and outcomes Ensure teams adhere to compliance, quality, and documentation requirements Cross-Functional Collaboration Work closely with Payor Relations, Operations, Quality, Analytics, and Technology teams Provide operational insight into payor attribution challenges and member behavior trends Support new VBC contracts or population expansions by designing outreach operating models Compliance & Patient Experience Ensure all outreach adheres to regulatory, contractual, and organizational requirements Monitor call quality and patient experience, especially during sensitive reengagement conversations Address systemic barriers to engagement uncovered through outreachSUPERVISORY RESPONSIBILITIES
Overseeing and managing the related Patient Engagement team which may include but is not limited to Patient Engagement Representatives, Team Leads, Supervisors, Managers, Training, and Quality Assurance.QUALIFICATIONS EDUCATION
Minimum of Bachelor's Degree required. 7 years of healthcare experience is commensurate to the education required.EXPERIENCE
Proven track record of developing and implementing successful patient engagement strategies.KNOWLEDGE, SKILLS AND ABILITIES
Extensive experience in value-based care operations, patient outreach, or population health Proven leadership of outreach, access, or call center teams Deep understanding of VBC attribution models, payor dynamics, and PCP alignment Skilled in population segmentation and targeted engagement strategies Strong data literacy and ability to partner with analytics teams Excellent leadership, communication, and change management capabilities Bilingual in English and Spanish strongly preferredTYPICAL WORKING CONDITIONS
