• Residential Recovery / PACE Detox Center Full-time X Part-time PRN Exempt X Nonexempt PACE Detox Center is a 24/7 residential withdrawal management program providing medically monitored detoxification services for adults with substance use disorders.

The Care Coordinator serves as the primary administrative, care coordination, and operational support position for PACE Detox Center. This position is responsible for coordinating client access, supporting admissions and discharges, facilitating continuity of care, maintaining administrative workflows, and assisting with day-to-day facility operations to ensure the program functions efficiently. The Care Coordinator works collaboratively with the Program Director, BH Clinical Team Lead, Charge RN, Administrative Access Supervisor, nursing staff, clinicians, residential staff, and community partners to promote timely access to care, safe transitions, and a high-quality client experience. This position also serves as the primary coordinator for many of the facility's administrative processes, including vendor coordination, maintenance requests, safety checks, inventory management, and administrative reporting. Duties and Responsibilities Client Access & Care Coordination Provides initial screening of client needs and assists individuals in accessing appropriate services. Coordinates admissions, discharges, and transitions of care. Completes Point of Entry (POE) processes including consents, releases of information, financial documentation, and intake paperwork. Coordinates appointments with providers, community partners, and aftercare resources. Maintains communication with referral sources to support continuity of care. Assists clients with transportation coordination and linkage to community resources. Updates daily tracking tools and systems for monitoring and tracking screenings, admission decisions, admissions, discharges, etc. Supports discharge planning by coordinating follow-up appointments and aftercare services. Monitors client appointments and follows up to promote continuity of care. Administrative Operations & Business Support Manages day-to-day administrative operations for the Detox program. Maintains accurate records and documentation within CareLogic. Coordinates MultiView purchase requests and tracks order completion. Processes invoices and assists with vendor payment tracking. Assists with monthly program reporting and operational data collection. Manages supply inventories while minimizing unnecessary expenditures. Oversees P-Card receipt collection and assists with reconciliation. Supports payroll, leave tracking, and Timekeeper responsibilities as assigned. Coordinates scheduling support and program communications. Answers telephones and provides outstanding customer service to clients, families, and referral partners. Facility Support & Safety Coordination Serves as the primary point of contact for facility vendors, maintenance contractors, and repair services. Coordinates facility maintenance requests through the MaintenX system and monitors completion. Assists with obtaining vendor quotes and coordinating repair services. Monitors office, medical, and unit supply inventories to ensure adequate stock. Coordinates monthly vehicle inspections, travel logs, fuel purchases, first aid kit inspections, and fire extinguisher checks. Performs routine facility safety inspections and assists leadership with monthly safety checks. Maintains Material Safety Data Sheets (MSDS) and assists with infection prevention and environmental safety practices. Supports facility readiness for HFRD licensing reviews, grant monitoring visits, and regulatory surveys. Assists with coordinating monthly safety drills and emergency preparedness activities. Program Support & Client Engagement Supports admissions and discharge processes by assisting clients during transitions into and out of the program. Provides direct client support during intake and discharge activities. Monitors client well-being and communicate concerns to nursing or clinical staff. Facilitates occasional psychoeducational or recovery-focused groups as assigned. Assists with client orientation and reinforces program expectations. Supports therapeutic activities and special program events. Team Collaboration & Continuous Improvement Participates in interdisciplinary treatment team meetings. Assists leadership with quality improvement initiatives and operational projects. Supports collection of program outcome data and grant reporting requirements. Identifies opportunities to improve workflows and client experience. Maintains positive working relationships with community providers and internal departments. Performs other duties as assigned.

Minimum Qualifications:

High School diploma/GED and three (3) years of job-related experience in a social services-related position; or One (1) year of experience at the lower level Social Svcs Tech 2 (SST011) or equivalent position. Ability to read, write, and speak English.

Preferred Qualifications:

Two (2) + years of experience in behavioral health field working with individuals with SPMI or SED or SUD. At least 2 years' experience providing care coordination in home services. Experience with CareLogic electronic health record system. Ability to practice a flexible range of skills in a fast-paced environment to support crisis situations and complex behavioral health needs Knowledge of community resources within Rockdale County. Other General:

Must be able to lift 20 pounds. Requires long periods of sitting/standing. Must have valid Georgia driver's license and Motor Vehicle Record in accordance with company policy. Candidates for selection must pass a criminal background check (including fingerprinting). Must pass a pre-employment drug screen and subject to random drug screens. Any combination of training and experience will enable the applicant to acquire the necessary knowledge, skills and abilities.
