What You'll Do As a Rural Health Transformation Program (RHTP) Regional Grant Coordinator, you will be embedded within one of Indiana's eight Rural Health Regions, serving as the on-the-ground connector between the Indiana Dept. of Health and the communities driving rural health transformation. You will coordinate with regional coalitions, hospitals, FQHCs, EMS agencies, and community-based organizations to advance grant-funded initiatives. Your work includes facilitating stakeholder meetings, tracking regional milestones, compiling performance data for CMS reporting, and identifying emerging needs within your region. This is a field-based role requiring deep community engagement, independent initiative, and a passion for improving health outcomes in rural Indiana. Candidates must reside within their assigned region.

Requirements:
What You Bring Experience with CMS-funded programs or federal cooperative agreements Familiarity with Indiana's rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations Experience collaborating with statewide associations and multi-stakeholder coalitions Knowledge of rural health delivery, care coordination, or health system transformation Strong communication, organizational, and community engagement skills Must reside within the assigned Rural Health Region At CSpring, we unlock the potential of people and data.
