Americare Senior Living is seeking an experienced MDS Coordinator to oversee the assessment and care planning process for our skilled nursing residents. In this role, you will complete and manage MDS assessments, ensure accurate documentation, and coordinate interdisciplinary care plans that support residents' safety, comfort, and quality of life. You will monitor regulatory compliance, optimize reimbursement, and mentor staff on assessment standards. Working in our resident-centered, compassionate, team-oriented environment, you'll build strong relationships with residents, families, and colleagues while supporting person-centered, dignified senior care.
