Job Description
MENTOR Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

- , you will play an essential role in supporting Adult Day Health participants by coordinating services, maintaining accurate documentation, assisting with admissions, and ensuring continuity of care across internal teams and community providers. - + Coordinate and oversee individualized participant programs to support clinical, social, and community-integration needs + Assist participants and families in understanding program goals, services, and processes + Serve as a primary point of contact for families, providing communication, resources, and support + Review evaluations, progress notes, and discharge documentation to ensure accuracy and coordinated care goals + Maintain complete and accurate participant records and documentation + Support discharge planning and follow-up coordination + Coordinate internal medical appointments and track required annual medical documentation + Communicate with healthcare providers, community resources, and external partners to promote continuity of care + Assist with training new staff involved in participant medical coordination, as needed •Qualifications•+ Bachelor's Degree in a social services-related field required + Two years of experience providing direct or indirect support services in a medical and/or rehabilitation setting - preferred not required + Experience working with individuals with disabilities preferred + Strong organizational skills and attention to detail + Excellent written and verbal communication skills + Compassionate, reliable, and responsible approach to supporting participants + Commitment to the organization's mission and values + Bilingual in English and Spanish required Join our team and help support participants, families, and communities through meaningful, person-centered care.
