Job Description
The Program Coordinator provides support and supervision to a team of House Managers, ensuring high-quality care is maintained in the home. The Program Coordinator will promote a positive atmosphere focused on growth and development by setting goals for each individual consumer, DSP, and House Manager. The Program Coordinator is responsible for coordinating, implementing, and tracking the performance management program for his/her team. The Program Coordinator's work is driven by the core values and mission of Golden Group Home Services.
Responsibilities Include:
- Works collaboratively with the GGHS Leadership Team, Direct Care Teams, and outside stakeholders.
- Supports, supervises, and provides guidance to a team of House Managers; works to continually enhance the professional development of all staff.
- Provides On-Call support.
- Ensures that House Managers and DSPs provide quality services and care to clients that meet HCBS standards.
- Ensures that medical and mental health needs and appointments as required are arranged.
- Provides guidance to direct care staff in the area of documentation, including but not limited to the review of client Individual Service Plans, Reportable Events and Incident Reports, and daily service notes. Ensures documentation is completed accurately and timely.
- Provides oversight of program and consumer goal planning and implementation.
- Maintains weekly work schedules which minimize overtime, meets parameters of authorized program hours, and approve weekly staff payroll.
- Facilitates and participates in monthly meetings with the House Managers and DSPs to address program and agency business.
- Distributes higher level agency information to staff such as policy revisions, critical updates, and other expectations.
- Maintains accurate and detailed documentation of all staff meetings and staff supervisions.
- Participates in employee evaluation and progressive disciplinary actions as needed.
- Maintains CRMA compliance, including ensuring current, accurate doctor orders and, in collaboration with the agency Nurse, meeting the health and safety needs of all residents. Completes annual DSP trainings, FA/CPR, and behavior management trainings as necessary.
- Completes data entry of Reportable Events in Evergreen in a timely manner, including the 30-day follow-up as required.
- Ensures all homes are in compliance with Federal and State regulations (licensing standards, MaineCare Section 21, etc.)
- Maintains ongoing guardian and case worker correspondence.
- Reports issues and concerns immediately to the appropriate person.
- Travel to the main office in South Portland as requested.
- Other Duties as assigned.
