- Full-time The Shared Living Coordinator is responsible for providing support, oversight, development, and guidance to assigned staff and contracted providers within the Shared Living Program.
The Shared Living Coordinator maintains communication with individuals served, families, guardians, case managers, community partners, and other stakeholders to ensure quality services and supports. The Shared Living Coordinator is responsible for promoting individualized, person-centered support options and collaborating with Shared Living leadership to support the continued growth and development of the program.
Essential Functions:
Individual and Provider Support:
Develop and maintain positive relationships with individuals supported and Contracted Home Providers to allow for effective observation, assessment, and support planning. Ensure all services and supports are aligned with the individual's Person-Centered Plan (PCP). Complete required home visits and contacts with Contracted Home Providers and individuals served in accordance with program standards and agency expectations. Maintain assigned caseload responsibilities as determined by Shared Living leadership. Provide individualized training and support to Contracted Home Providers as needed. Establish and maintain professional working relationships with individuals supported, families, staff, providers, and community partners.
Program Oversight and Compliance :
Ensure Contracted Home Providers provide quality services and care to individuals served in accordance with state and federal regulations, Shared Living program requirements, and 3Rivers policies and procedures. Work collaboratively with Contracted Home Providers, in-home Direct Support Professionals, Case Managers, and interdisciplinary team members to ensure documentation is completed accurately and timely in accordance with Medicaid and Home and Community-Based Services (HCBS) requirements. Maintain accurate records for individuals served and Contracted Home Providers to support regulatory compliance and best practices. Follow all reporting requirements and agency procedures regarding incidents, safety concerns, abuse/neglect allegations, and other events affecting the health and safety of individuals served, staff, providers, or the community.
Planning and Team Collaboration:
Participate in Person-Centered Planning meetings, Service Implementation Plan (SIP) meetings, and other team meetings as required. Collaborate with Shared Living leadership to support program growth, quality improvement, and individualized service development. Communicate effectively with agency departments, community partners, and external stakeholders to support coordinated care and services.
Recruitment and Provider Development:
Assist with recruitment, screening, onboarding, monitoring, and evaluation of Contracted Home Providers. Support the development and retention of quality Shared Living providers through ongoing communication, training, and guidance. Physical Requirements:
Ability to remain in a stationary position (sitting or standing) for extended periods. Ability to move throughout program facilities, residences, and community settings. Occasionally lift or carry up to 35 pounds (e.g., supplies, documents, equipment). Ability to bend, squat, and assist in emergency situations if necessary. Must have reliable transportation and be able to travel throughout assigned region, as well as other agency regions as needed.
Qualifications:
Bachelor's degree in Social Services or a related field preferred. Equivalent experience may be substituted for education. Experience working with individuals with intellectual and developmental disabilities preferred. Possess excellent verbal and written communication skills. Demonstrate the ability to work independently in an environment requiring flexibility and management of multiple priorities. Possess professional values and ethics and maintain confidentiality in accordance with federal, state, and agency requirements. Demonstrate working knowledge of Microsoft Office programs, including Word and Excel. Demonstrate the ability to motivate others, manage conflict professionally, and accept constructive feedback. Be a team player with a positive attitude and strong interpersonal skills. Valid and insurable Maine driver's license required. Ability to pass required background checks, to include: Adult/Child Protective Services; criminal history; OIG Exclusion list; National Sex Offender Registry; Bureau of Motor Vehicles record; and Maine CNA Registry.
