We specialize in culturally responsive care and support individuals with disabilities in living more independently and safely in their communities. We are committed to quality, compassion, and integrity in everything we do. Position Summary We are seeking a full-time Program Coordinator to fulfill the responsibilities of a Designated Coordinator under Minnesota 245D licensing standards. The ideal candidate is highly organized, and experienced in managing service coordination, compliance documentation, and client support under the 245D waiver program. This is a non-direct care administrative position. Key Responsibilities Lead and manage the intake process for new clients, including gathering required documentation and coordinating initial service setup Coordinate with case managers, families, and staff to ensure a smooth transition from intake to active service delivery Develop and maintain CSSP Addendums, IAPPs, risk management plans, and other 245D-required documentation Monitor deadlines and ensure timely completion of assessments, progress reviews, and service updates Maintain accurate, confidential records in accordance with HIPAA and licensing standards Ensure client outcomes and support strategies are implemented and documented properly Respond to service changes, incidents, and support plan updates as needed Collaborate with the Designated Manager to ensure ongoing compliance and quality service delivery Communicate regularly with clients, families, and external partners to support continuity of care Participate in internal audits and prepare for licensing visits Assist with general administrative tasks as assigned, supporting smooth daily operations of the program Use person-centered and culturally responsive approaches in all interactions Required Qualifications Bachelor's degree in a human services field + 1 year of full-time direct care experience OR Associate's degree or diploma in a human services field + 2 years of direct care experience OR High school diploma/GED + 3 years of direct care experience in a human services setting Strong understanding of 245D service coordination and documentation Excellent communication, organization, and time management skills Comfortable using computers, digital forms, and electronic documentation systems Valid driver's license and access to a reliable vehicle Must pass a DHS background study and meet all applicable 245D provider requirements Preferred Qualifications Bachelor's degree in a human services field 2+ years of experience overseeing 245d services Experience working with diverse or multilingual client populations Candidates with experience in program coordination, service planning, care coordination, or case management are strongly encouraged to apply.
