$23.00-$27.00 per hourPosition Type:
Full-Time Lakes Homes and Program Development, Inc. is seeking a motivated and professional Designated Coordinator (DC) to provide leadership and oversight within our 245D Community Residential Services (CRS) programs serving adults with disabilities. The Designated Coordinator plays an important leadership role in supporting staff, ensuring quality services, maintaining compliance with Minnesota 245D regulations, and promoting person-centered supports that enhance the quality of life for individuals served.Our mission is:
Enhancing Human Worth and Potential Position Summary The Designated Coordinator (DC) provides direct operational leadership, staff supervision, and program oversight for assigned Community Residential Services homes. This position supports Direct Support Professionals (DSPs), coordinates services for individuals served, assists with staffing and scheduling needs, ensures documentation and compliance standards are met, and works closely with guardians, case managers, nursing staff, and leadership. This role requires strong communication skills, professionalism, organization, and the ability to lead staff in a positive and supportive manner. Responsibilities Include Overseeing day-to-day operations of assigned residential homes Supervising and supporting Direct Support Professional (DSP) staff Assisting with onboarding and staff training Ensuring compliance with Minnesota 245D regulations and company policies Monitoring documentation, MARs, incident reports, and service implementation Supporting individuals served in achieving person-centered goals Communicating with guardians, case managers, medical providers, and team members Assisting with staffing coordination and schedule support Participating in on-call rotation and responding to program needs as necessary Promoting a safe, respectful, and professional environment QualificationsRequired High school diploma or GED Valid driver's license with acceptable driving record Ability to pass Minnesota DHS background study requirements Strong communication, leadership, and organizational skills Ability to work independently and as part of a team Basic computer and documentation skills Preferred Associate degree or higher in Human Services, Social Work, Psychology, Healthcare, Special Education, or related field preferred Minimum of two (2) years of supervisory or leadership experience preferred Experience working in Minnesota 245D services or related human services settings preferred Knowledge of Minnesota 245D regulations and compliance standards preferred Experience supporting individuals with developmental disabilities, mental health diagnoses, behavioral needs, or medical complexities preferred Experience with electronic documentation systems such as Therap and scheduling systems such as Deputy preferred Benefits Include Paid Time Off (PTO) ESST Holiday pay Health insurance options Paid training Professional development opportunities Lakes Homes and Program Development, Inc. is an Equal Opportunity Employer.Enhancing Human Worth and Potential Pay:
