Job Description
Description Lakes Region Community Services is currently accepting applications for a full-time (37. hours per week) Service Coordinator to coordinate supports and services for individuals with developmental disabilities or acquired brain injuries and their families. Duties include but are not limited to: facilitate team meetings, write service agreements, provide advocacy, act as a community liaison, develop natural supports, and coordinate referrals/connections to other programs within LRCS. Service Coordinators are also responsible for assisting in necessary assessments, providing direct support as needed, and participating in agency, community and state committees as appropriate. Requirements Bachelor's degree, Associate degree or Human Services Certificate with 2-4 years of directly related experience in a field such as Human Services, social work, education, etc. is preferred. A combination of relevant work experience and credits toward a degree program may be considered at the discretion of the Agency. Valid NH Drivers' license, reliable vehicle, and agency-determined minimal motor vehicle insurance coverage are required.
