Job Description

Position Summary The Program Coordinator coordinates community outreach initiatives that raise awareness of healthy living, connect service-area residents to essential preventive medical services, and facilitates seamless navigation through screenings and preventive care. Primary Position Responsibilities 1. Provides outreach activities to promote cancer prevention awareness and screenings. 2. Coordinates community events & connects people to needed resources. 3. Prepares & distributes educational materials that promote healthy lifestyle and positive health outcomes. 4. Assists with data collection for grant reporting and renewals making sure grant deliverables are met or exceeded. 5. Assists patients with navigating their healthcare and screenings. 6. Actively seeks strategies to reach residents experiencing barriers to access to care..
Qualifications Minimum Education:
Required:
High School Diploma Preferred:
Bachelor's Degree in Health Education, Public Health, Nursing or related degree. Minimum Years of Experience (Amount, Type and Variation):
