Job Title:
Program Coordinator of Higher Level of Care Services Overview:
Plan direct and coordinate assigned program activities in compliance with agency's policies and procedures. Responsibilities include, but not limited to: 1. Assist the Clinical Supervisor and Director of Clinical Services with the oversight of day-to-day program operation of the assigned staff to ensure quality service delivery in a client centered strength based integrated care approach using illness management and recovery concepts. 2. Assist Clinical Supervisor / Director of Clinical Service in increasing services efficiency and effectiveness by assign tasks to direct care staff as necessary i.e. group schedule and programming is implemented in accordance with schedule, break coverage, crisis management. 3. Proactively recommend and implement improvements in process, service delivery and program operations based on outcome measures of consumer satisfaction surveys 4. Ensure a safe and supportive environment for patients, staff and visitors 5. Facilitate and participate in staff orientation and training for new and existing staff. 6. Act as liaison with other operational departments within the agency, clients, and outside providers and educate about services provided during care coordination and outreach contacts. 7. Participate in weekly staff meetings and clinical conferences. 8. Provide assigned staff with professional administrative and clinical care mentoring to support agency mission of quality services and risk management. 9. Monitoring ongoing quality assistance standard, collect data and assist Clinical Supervisor and Director of Clinical Services in preparation of monthly and quarterly QA reports. 10. Provide coverage as needed including modification of work schedule or working extra hours as the situation may require. 11. Provide individual, family and group treatment for case load as assigned in accordance with job description for Clinical Therapist 12. Interact with staff, clients and outside providers with respect and courtesy while maintaining confidentiality. 13. Other duties as assigned.
Requirements:
Master Degree in Social Work, Psychology Counseling, or Counseling related field. Must hold current and valid license LPC, LCSW, or LCADC. Minimum 1 year of staff supervisory experience. Must possess excellent communication skills (written, verbal), basic computer proficiency, motivational interviewing skills exemplary organizational, problem-solving, decision-making and documentation skills. Ability to motivate, work as a team member although independently, utilize excellent judgment skills
