JOB SUMMARY
The HVIP Program Coordinator is responsible for coordinating daily program operations across University Hospital (UH) and Newark Beth Israel Medical Center (NBI). The position coordinates Community Health Workers (CHWs), manages program data and reporting, monitors operational activities, and maintains communication with hospital personnel, NCST leadership, and community partners. The coordinator ensures consistent program implementation, accurate documentation, timely reporting, effective communication, and follow-through across both hospital sites. This position works under the direction of the Director of HVIP and is accountable for maintaining organized, efficient, and responsive program operations.
Essential Duties and Responsibilities:
1. Multi-Site Program Coordination Coordinate daily HVIP operations across University Hospital and Newark Beth Israel. Monitor program activities, referrals, participant follow-ups, and outstanding operational tasks at both sites. Maintain consistent workflows, documentation procedures, and program standards across hospital locations. Coordinate hospital meetings, on-site activities, and communication with designated hospital contacts. Identify operational barriers, service gaps, and site-specific needs and communicate them to NCST leadership. Maintain trackers for assignments, deadlines, meetings, and unresolved concerns.
• Provide regular updates on program activities, challenges, and outcomes across both locations. 2. Community Health Worker Coordination Provide day-to-day operational coordination and support to CHWs assigned to both hospital sites. Conduct weekly check-ins to review caseloads, participant needs, service plans, documentation, and follow-up responsibilities. Monitor completion of assigned tasks and communicate outstanding responsibilities to CHWs. Reinforce NCST policies, hospital protocols, documentation standards, and professional expectations. Support CHWs in coordinating participant services and addressing barriers to care. Identify recurring workflow, communication, or performance concerns and escalate matters requiring supervisory intervention to NCST leadership. Ensure CHWs receive timely updates regarding program procedures, hospital expectations, and operational changes. 3. Data Management and Reporting Manage HVIP data collection, data entry, data quality assurance, and reporting across both hospital sites. Maintain accurate, complete, and timely participant and program records in designated systems. Monitor documentation and data entry for missing information, inconsistencies, and overdue records. Develop and maintain data trackers, caseload reports, service utilization reports, and program dashboards as needed. Track participant encounters, referrals, service connections, follow-up activities, and program outcomes. Prepare weekly reports distinguishing UH and NBI activity while providing combined program totals. Compile reports for NCST leadership, hospital partners, and funders as assigned. Identify trends, reporting discrepancies, and service gaps and communicate findings to NCST leadership. Maintain confidentiality and compliance with applicable data security and recordkeeping requirements. 4. Participant Services and Care Coordination Support CHWs in coordinating participant access to healthcare, behavioral health, housing, transportation, employment, and community resources. Monitor referrals, service plans, and follow-up activities to support continuity of care. Help identify barriers to participant engagement and coordinate appropriate referrals and internal support. Support communication between participants, CHWs, hospital personnel, and community providers while maintaining confidentiality and participant choice. Escalate participant-related concerns requiring clinical, supervisory, or executive decisions to the appropriate personnel. 5. Hospital and Community Partner Communication Maintain professional working relationships with University Hospital, Newark Beth Israel, NCST personnel, and community partners. Attend assigned hospital and partner meetings. Communicate program updates, operational needs, and action items to relevant stakeholders. Follow established communication protocols and supervisory structures when addressing hospital or partner concerns. Provide written follow-up on significant meetings and operational discussions, including decisions, assigned responsibilities, and outstanding matters. Coordinate communication between hospital sites when operational issues affect both locations. Promptly communicate concerns involving participant services, CHW assignments, hospital operations, or interagency coordination to the appropriate NCST supervisor.
Knowledge, Skills, and Abilities:
Strong organizational skills and ability to manage multiple priorities across two hospital sites. Excellent written and verbal communication skills. Strong data management, reporting, documentation, and quality assurance skills. Proficiency in Microsoft Office Suite, Google Workspace, databases, and related reporting tools. Ability to follow established procedures, supervisory direction, and communication protocols. Strong interpersonal skills and ability to work effectively with hospital staff, CHWs, community partners, and participants. Knowledge of healthcare coordination, community resources, and barriers to care. Demonstrated professionalism, accountability, sound judgment, attention to detail, and follow-through. Understanding of trauma-informed, survivor-centered, and culturally responsive services.
