KEY RESPONSIBILITIES 1.
Housing Supports Program — Case Management & Enrollment Growth Serve as the primary case manager for all NJ Family Care Housing Supports Program clients, completing assessments, developing individualized housing stabilization plans, and delivering required touchpoints per MCO authorization requirements. Conduct Level of Need Assessments for all enrolled clients; maintain documentation in HMIS consistent with DMAHS requirements. Actively identify and enroll eligible clients, including current reentry residents and prior program participants. Build and maintain working relationships with all five credentialed MCO partners (Aetna, Fidelis Care, Horizon, UnitedHealthcare, Wellpoint) to ensure timely authorizations, accurate monthly billing, and clean claims. Coordinate warm handoffs, reauthorizations, and transitions as clients achieve housing stability. 2. Reentry Housing Programs — Oversight (Orange & Edison) Oversee day-to-day operations of both reentry homes, including the NJLEAD program and NJ Youth Justice Commission Transitional Housing Grant, ensuring compliance with all grant requirements, occupancy targets, and resident program expectations. Manage bed assignments, resident onboarding, and transitions; maintain occupancy records and report to the CEO. Ensure all grant-eligible beds are occupied and that monthly billing is submitted accurately and on time. Coordinate with property owners/managers on maintenance and facility matters at both locations. Foster a trauma-informed, supportive environment in both homes, establishing clear expectations and accountability structures for residents. Track resident outcomes including housing retention, employment, program completion, and recidivism indicators. 3. NJLEAD Grant Program — Compliance & Reporting Manage all programmatic compliance requirements of the NJLEAD grant, including data tracking, outcome reporting, and deliverable timelines consistent with the grant agreement. Coordinate wraparound supports for NJLEAD participants including peer navigation, transportation assistance, and Healing Circles. Prepare and submit required grant reports to the funder; maintain organized records of program activities, participant files, and expenditures. Maintain collaborative relationships with

representatives, respond to compliance review requests promptly, and identify opportunities for continued or expanded grant funding. 4. Brick City Candy — Operational Oversight Provide management oversight of Brick City Candy operations, ensuring the social enterprise functions as both a viable business and a meaningful reentry employment and training program. Coordinate production schedules, inventory, and fulfillment, working with reentry participants engaged in the enterprise. Support marketing and sales efforts, including business relationships developed through media exposure and community partnerships. Track business performance metrics and report to the CEO; identify opportunities to grow revenue and expand participant engagement. 5. Administration, Data & Compliance Maintain accurate participant records across all programs; ensure confidentiality and HIPAA compliance. Prepare regular program performance reports for the CEO covering all program areas. Support budget monitoring; flag variances and contribute to financial sustainability planning. Participate in community coalitions, funder meetings, and public events representing the organization's programs.
QUALIFICATIONS
Required Bachelor's degree in social work, human services, public health, criminal justice, nonprofit management, or a related field; OR equivalent combination of education and direct experience. Minimum 3 years of experience in program management, case management, or social services, with demonstrated supervisory or leadership responsibilities. Knowledge of the reentry population and the barriers faced by individuals returning from incarceration, including housing instability, employment barriers, and behavioral health needs. Demonstrated ability to manage multiple programs simultaneously with strong organizational skills and attention to detail. Experience with Medicaid-funded programs or managed care billing strongly preferred; candidates with comparable regulated service delivery experience and a demonstrated ability to learn complex compliance systems quickly will also be considered. Proficiency in case management documentation, data tracking, and reporting; experience with HMIS or comparable database systems strongly preferred. Valid driver's license and reliable transportation for travel between Orange and Edison sites. Strong written and verbal communication skills; ability to represent the organization professionally with funders, MCO partners, and community stakeholders. Preferred Master's degree in social work (MSW), public administration, or related field. Licensed Social Worker (LSW or LMSW) or eligibility for licensure in New Jersey. Experience with NJ Family Care / Medicaid Housing Supports Program or similar Housing First models. Experience managing or supporting a social enterprise or workforce development program. Lived experience with the criminal justice system — we strongly encourage individuals with personal reentry experience to apply. Bilingual (English/Spanish) a plus.
