•On-site office work Monday-Friday either 8am-4pm or 9am-5pm - On-site corporate office's Parsippany location. After training 1 day work from home is available. Responsible for authorizing, supervising, and providing care to adults with serious mental illness. Oversees the administration and daily operations of the Community Support Services (CSS) program and supervises program employees. Conducts comprehensive rehabilitation needs assessments. Develops, implements, monitors, updates, and approves individualized rehabilitation plans. Provides therapeutic rehabilitation and skill-development services. Delivers services based on each consumer's needs. Provides crisis intervention and 24-hour on-call support to employees and consumers, including emergency service referrals. Submits care recommendations to the State Interim Management Entity (IME) for authorization. Coordinates, supervises, and delivers CSS services, including assessments, evaluations, rehabilitation planning, case conferences, referrals, family meetings, group services, individual counseling, telehealth, advocacy, community-agency coordination, and discharge planning. Ensures compliance with agency billing and fiscal procedures. Supports program development, new initiatives, marketing, and community engagement. Supervises program employees, including orientation, training, performance evaluations, and professional development. Monitors consumer self-administration of prescribed medications according to agency procedures. Connects consumers with community-based wellness and recovery services. Ensures compliance with local, state, federal, accreditation, and agency quality-assurance standards. Transports consumers safely in agency vehicles as needed. Completes service documentation accurately and promptly while protecting consumer confidentiality.

QUALIFICATIONS

Must have either: NJ Licensed Clinical Social Worker, NJ Licensed Professional Counselor, OR NJ Registered Nurse. Valid NJ driver's license, with a good driving record. VEVRAA Federal Contractor.
