Job Description
Heart to Heart is seeking a compassionate and dependable Lead Direct Support Professional to guide our Day Program and support individuals with Intellectual and Developmental Disabilities in achieving their ISP goals and coordinate daily programming and staff assignments, while building meaningful relationships with the individuals and staff and to create a warm, safe and inclusive environment where every individual can thrive. Non Probationary Driver's License Two (2) years lead staff or management experience, preferred High School Diploma or Equivalent Organizational skills and attention to detail Experience with Therap, preferred Microsoft Office Suite experience All DDD required trainings Develop and maintain interactive relationships with individuals that reflects the values of Heart to Heart Participate in ISP meetings and may assist in development of ISP Oversee implementation of ISP Create goal sheets for each individuals ISP with Manager or Director Coordinate service provisions within the community in conjunction with Day Program Manager/Support Coordinator's Create housekeeping assignments for DSP staff Plan and implement recreational activity plans for the individuals with adherence to activity schedule Identify and implement strategies for maximizing the inclusion of individuals Administer medication and document such in compliance with Heart to Heart policies and procedures and DDD guidelines Orient new individuals to basic safety practices and ensure emergency drills are conducted as required Transport individuals to community destinations in Heart to Heart vehicles and supervise individual during outings Follows through with additional responsibilities and tasks as related to care and compliance as assigned by Management Maintain individuals hygiene, dietary needs and restrictions Assist with training new staff
