Job Summary:

The Care Facilitator is responsible for assisting the Consumer/Member in the approval process for the Private Services division of the Agency. She/he is knowledgeable of Medicaid program regulations.

Duties and Responsibilities:

May serve as an interpretive liaison between Third Party Assessors and Consumer/Member per Consumers/Members request. Perform scheduled supervisory visits to Consumer/Member, in their home, on a monthly basis per State Program guidelines. Assist with internal and external transfer process. Responsible for accurately documenting changes in Consumer/Member information. Produce Incident Reports when needed. Verify Consumer/Member eligibility and authorizations prior to admission and thereafter as directed by State Program Guidelines. Participate in after hours and weekend On-Call rotation. Prepare for State and Agency audits.

Requirements :

At least a high school diploma. A minimum of 1 year of experience in working in an office environment, is preferred. Ability to provide proof of right to work in the United States. Proficiency in Microsoft office including but not limited to Excel and Microsoft word. Must have a current driver's license and clean driving record to get on company insurance policy. Willing to travel as deemed necessary. Ability to pass a criminal background check.
