Job Description

Position Summary The Resource Navigator plays a vital role in mitigating food insecurity and poverty in the City of Santa Fe. This position coordinates and delivers wraparound support services for Kitchen Angels' clients, including
SNAP/WIC
enrollment assistance, resource navigation, and community referrals. The Resource Navigator also represents Kitchen Angels at outreach events and strengthens relationships with community partners to ensure clients receive comprehensive support. The ideal candidate is compassionate, bilingual in English and Spanish, experienced in client-centered service, and capable of working with diverse populations. Key Responsibilities Client Navigation & Outreach Screen and process telephone or walk-in inquiries; Coordinate In-takes; Assess client needs beyond meal delivery and assist with accessing social, medical, and economic resources (e.g., transportation, housing, utilities, health care, personal safety, and benefits enrollment); Guide clients through enrollment in public benefits programs, such as SNAP, WIC, and Medicaid; Obtain and document client consent and intake data using written and electronic tools; Track specified metrics; Use Unite Us software (training provided) and Kitchen Angels' internal client resource management database to document, track, and follow up on referrals and service outcomes; Provide culturally and linguistically responsive communication to clients, including homebound individuals and families in crisis. Partnerships & Community Engagement Develop and maintain strong working relationships with partner organizations within the CONNECT network and the broader Santa Fe community; Actively promote Kitchen Angels' services at health fairs, community events, and presentations—including occasional Saturday events; Coordinate with local providers to encourage referrals through Unite Us and other communication channels; Represent Kitchen Angels with professionalism, integrity, and care in all community and partner-facing settings; Work collaboratively with other departments. Client Services Support Process new client applications and update client records; Support the intake process and help ensure timely and accurate client enrollment and reassessments; Assist with reporting requirements; Triage clients; Conduct month-end archival reconciliation. priority through accurate data collection, effective use of technology, consistent procedures, and strong cross-department coordination. Track and report information that helps Kitchen Angels evaluate client outcomes, client satisfaction, referrals and referral outcomes, service quality, number of clients served, and rural reach. Contribute to Safety Net and Navigation Services by strengthening comprehensive client intake and assessment, coordinated referrals, and partnerships that address the broader needs of aging and medically fragile neighbors. Participate in continuous improvement using client feedback, service data, and "What Works" performance measures. Qualifications Education & Experience Associate degree in human services, social work, public health, or a related field preferred; Minimum of 2 years' experience in case management, client navigation, or community outreach; Demonstrated experience working with culturally and linguistically diverse populations. Skills & Competencies Excellent interpersonal and communication skills; Bi-lingual a plus, but not required; Continued education; Proficiency with computer systems and willingness to learn new software platforms (e.g., Unite Us, Salesforce, or other CRM tools); Knowledge of Santa Fe-area community resources and public benefits systems a plus; Commitment to client dignity, confidentiality, and trauma-informed service delivery. Working Conditions & Requirements Ability to sit and work at a computer for extended periods; Ability to lift up to 50 pounds; Moderate exposure to video display terminals (VDT); Must have access to reliable transportation for travel within Santa Fe County and occasional trips outside of the area; Must possess a valid New Mexico Driver's License and current vehicle insurance; Must pass a criminal background check; Must be at least 21 years of age; Respond to internal and external communication per policy.
Pay:
$22.00 - $26.00 per hour Expected hours: 40.0 per week
