Job Description
Sun River Health provides the highest quality of comprehensive primary, preventative and behavioral health services to all who seek it, regardless of insurance status and ability to pay, especially for the underserved and vulnerable. Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County. Sun River Health is currently seeking a part-time Genesis Care Specialist to join our team in Coram, NY. The Genesis Care Specialist for the HIV program maintains the roles and responsibilities of a Genesis/HIV care manager, provides support to the other care managers, oversees QI and CQI for their site and works closely with the Program Coordinator and Director to assure that the program is responsive to the Ryan White and other grant requirements, and related agency/program initiatives. Additionally, the Genesis Care Specialist attends case conference meetings to assure that the continuity and consistency of services are provided in a timely manner to HIV+ patients, and that care managers are able to cover one another's patients during absences. The Genesis Care Specialist also can assist in the supervision of program peers and Genesis Care Managers when applicable. The Genesis Care Specialist orients social work interns, peers and care managers to the site, provides appropriate intervention, support and problem solving with staff and patient issues, assists with the ongoing development of community-based resources, and relationship building with collateral agencies.
