Service Coordinator

About Us The Head Injury Association is currently looking for a Service Coordinator for the Traumatic Brain Injury waiver program. This position will report to the Director of Community Based Services and oversee a caseload of up to 25 individuals within the Long Island region. Job Description Working within Nassau and Suffolk County, the Service Coordinator's primary responsibility is to work in partnership with the individuals we serve to enable them to live and participate in the community as independently as possible. Service Coordinator will assist with benefits and entitlements, annual plan writing, monthly note completion, conducting team meetings and visiting individuals within their homes regularly to assure quality of care.
