Non-Exempt Job Description:
The 1115 Waiver Program Coordinator is responsible for client assessment, needs fulfillment, food delivery and record keeping for this program. This is a Full-time position of 40 hours per week.
Essential Functions:
Conduct initial intake/assessment with members. Review referrals and confirm member insurance status and confirm enrollment in appropriate program. Put together meal boxes according to client's assessed needs. Schedule delivery of meal boxes to client's homes or LIC satellites for pickup. Determine eligibility for cooking supplies and fulfill authorized distributions. Keep detailed records of client assessments and deliveries in database. Collect and retain proof of delivery documentation. Submit appropriate paperwork for reimbursement. Oversee invoice approval and payment. Conduct member outreach. Perform other duties and tasks as assigned by the Director of Nutrition & Community Health Programs and VP of Network Relations and Capacity Building.
Education & Experience:
Fluent in Spanish and English. Excellent verbal and written communication skills. Excellent interpersonal and customer service skills. Excellent organizational skills and attention to detail. Excellent time management skills with a proven ability to meet deadlines.
Required Skills & Abilities:
Bachelor's degree preferred but not required. Relative work experience in office operations, non-for-profit, or human services preferred.
Physical Requirements:
Prolonged periods of sitting at a desk and working on a computer. Valid and clean NYS driver's license. This position is funded by a grant that is currently secured through March 2027. While Long Island Cares intends to seek renewal of this grant annually, continued funding for this role is not guaranteed. Employment beyond the current grant period is contingent upon securing additional funding.
