Position Summary:

The Service Coordinator (SC) provides case management to participants and prospective participants of the Home and Community Based Service/Traumatic Brain Injury Waiver Program of New York State (HCBS/TBI Waiver). The SC coordinates and monitors all of the services in the participant's Service Plan, which may include Medicaid State Plan Services, as well as educational, vocational, social and medical services. The SC works with the participant to increase their independence, productivity and integration into the community while maintaining their health and welfare. Prepare and submit all required documentation and semi-annual service reports in a timely manner, in alignment with Department of Health (DOH) and waiver program guidelines. Ensure reports are shared with the appropriate staff and agencies. Keep accurate and well-organized records for each waiver participant, maintaining service coordination files in accordance with Schofield Home Care's policies and applicable regulatory standards. Conduct monthly in-person visits with participants, and additional visits as needed, to provide support, gather updates, and assess satisfaction with services. Maintain thorough documentation of all participant contact, including face-to-face meetings, phone calls, and emails. A minimum of one quarterly visit must occur in the participant's home, or more frequently if required. Organize and lead Clinical Team Meetings based on each participant's approved Service Plan. Ensure representation from all service providers is secured, either in person or via phone/video, and that participation is properly documented. Protect participant confidentiality at all times, following HIPAA regulations and Schofield Home Care's internal privacy practices. Monitor and manage supplies and equipment used in support of service coordination activities. Take on other duties as needed to support the goals of the program and the needs of participants. srpp Education and Training To qualify for providing service coordination to participants in the TBI Waiver Program, candidates must meet one of the following five education/training and experience combinations: Hold one of the following professional degrees or certifications: Master of Social Work (MSW) Certified Rehabilitation Counselor (CRC) Registered Occupational Therapist (OTR) Registered Physical Therapist (PT) Registered Speech-Language Pathologist (SLP) Registered Professional Nurse (RN) Master's or Doctorate in Psychology Certified Special Education Teacher In addition, these individuals must have at least one (1) year of experience providing service coordination and offering information, linkages, and referrals to community-based services for individuals with disabilities. OR Hold a Bachelor's degree (BA) and have at least two (2) years of experience providing service coordination and connecting individuals with disabilities to community-based services. OR Hold an Associate's degree and have at least three (3) years of experience in service coordination and community-based referrals for individuals with disabilities. OR Hold a High School Diploma or GED and have a minimum of four (4) years of relevant experience in service coordination and referrals for community-based services. OR Be an individual who has previously served successfully as a Regional Resource Development Specialist (RRDS). Character - The individual must possess good moral character, patience and compassion for the chronically ill or disabled. Must possess a valid driver's license and have a car available for use.
