Job Description

The Program Coordinator (Qualified Intellectual Disabilities Professional (QIDP)) is responsible for coordinating, monitoring, and evaluating services provided to individuals with intellectual and developmental disabilities residing in OPWDD cert. sites.
PURPOSE OF THE POSITION
The Program Coordinator (Qualified Intellectual Disabilities Professional (QIDP)) is responsible for coordinating, monitoring, and evaluating services provided to individuals with intellectual and developmental disabilities residing in OPWDD-certified residential programs. The QIDP ensures that services are person-centered, compliant with OPWDD regulations, and designed to promote independence, community inclusion, health, safety, and quality of life.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Actively promotes the mission, vision and values of Empower. Ensure that residential programs promote person-centered approach in service planning and delivery. Lead the development, implementation, and review of CFA's , IPP and SAP's. Ensure services and supports are tailored to each individual's strengths, preferences, needs, and desired outcome Coordinate interdisciplinary team meetings and facilitate communication among all service providers. Monitor the delivery of residential services to ensure quality and effectiveness. Conduct regular reviews of documentation, progress notes, and goal tracking. Ensure all services are implemented in accordance with approved plans and regulatory requirements. Serve as a liaison between individuals, families, guardians, and service providers. Maintain regular contact with individuals, residential teams, day/vocational programs, clinicians, families, and external stakeholders. Chair and participate in all annual, semi-annual and special team conferences. Complete and maintain all required documentation (i.e., minutes, program plans). Ensure monthly notes are completed correctly and contemporaneously and that the records are maintained accurately for all people who receive services. Maintain systems, policies, and protocols that represent best practices Actively participate in intake and placement process. Review Life Plans to ensure that support and services are accurately reflected. Coordinate weekly clinical meetings for ICF and biweekly for IRA's. Complete UR forms as needed under the supervision and guidance of the supervisor. Complete and submit daily PC reports. Ensure compliance with
OPWDD /OMIG
regulations, Medicaid requirements, agency policies, and residential program standards (DOH and CMS). Ensure Medicaid billing compliance for services delivered. Actively Participate in audits, surveys, and quality assurance reviews. Participate in agency quality improvement committees and initiatives. Responsible for completing all training as assigned.
SUPERVISORY RESPONSIBILITIES
Possess strong commitment to team environment dynamics with the ability to contribute expertise and leadership at appropriate times. Participate and contribute to Department meetings and training. Provide guidance and consultation to Direct Support Professionals (DSPs), Residential Managers, and other program staff. Foster a collaborative interdisciplinary team approach. Conduct and maintain records for staff training related to CFA, IPP, IPOPS, SAP, person-centered planning, and regulatory requirements. Promote self-advocacy for people we support and advocate for individual's rights and choices. Attend all mandated training courses and events. Provide temporary leadership coverage to residences in the absence of Residential Managers. All other related duties as assigned.
Incident Management and Prevention:
Assist and cooperate with the agency and Justice center investigative process related to incidents. Coordinate with Residential managers and supervisors with Incident management (notifications, Meetings, staff training, CAPS) Review incident reports, safeguard concerns, and corrective actions to ensure appropriate follow-up. Assist in developing and implementing corrective action measures. Ensure health and safety concerns are addressed promptly
QUALIFICATIONS
Bachelor's degree in health /human services field and a minimum of one (1) years' experience working with developmentally disabled individuals. Knowledge of
OPWDD HCBS
Waiver and ICF regulations. Must be able to provide weekend coverage as scheduled and work flexible hours as needed. Good knowledge of Microsoft Office Applications and familiarity with electronic medical record applications (EMR) Valid driver's license (strongly preferred) or ability to travel between program sites Performs all job responsibilities in full compliance with all applicable laws, rules, regulations, policies and procedures. Adheres to the agency's corporate compliance program, including the code of business conduct, the NYS and federal false claims acts, whistleblower protections and mandatory reporting. Completes corporate compliance training at least annually. Fully cooperates in all corporate compliance investigations and reviews.
