Job Description

We are seeking an At-Risk Navigator in Trumbull County. Responsibilities include, but are not limited to: Providing coordination of care, advocacy and outreach to post hospital, jail and crisis stabilization unit consumers within timeframe established. Coordinating and/or assisting in crisis management and stabilization of the consumers served, including participation in development and review of crisis plan,their treatment plan and reinforcing healthy behaviors with the consumers. Providing phone and in-person follow up in the Crisis Center and in the community.
Rate:
$21.00 per hour Must have bachelor's or master's degree in social services or related field. In addition, must have experience working inpatient or outpatient programs with people who are severely mentally disabled and/or dually diagnosed, knowledge of psychiatric disorders, and significant familiarity with community systems and agencies. Must be willing to complete outreaches in the community. Must have and maintain excellent driving record. Prior experience working with clients in a community-based setting is preferred. Must be very organized and have good familiarity with excel documents Must have or be willing to obtain NPI and Medicaid Provider numbers.
