Preferred:
  • Bachelor's degree in Social Work, Human Services, Psychology, or related field
  • 2+ years of case management or case coordination experience
  • Experience working with older adults, veterans, or individuals with disabilities
  • Familiarity with Medicaid, OPI-M, or Veteran-Directed Care program
  • Manage complex caseloads independently

Summary:
Seeking a skilled and compassionate Case Manager to provide client-center services to older adults and veterans. This position is responsible for eligibility determination, needs assessment, service coordination, and compliance-driven documentation in alignment with the Older Americans Act (OAA) and Oregon DHS/APD requirements.
Program Allocation:
  • 50% Veteran-Directed Care (VDC)
  • 25% Oregon Project Independence-Medical (OPI-M)
  • 25% Additional programs
Duties Include:
  • Conduct intake and eligibility screenings
  • Perform comprehensive needs assessments
  • Develop and monitor service plans
  • Coordinate services with community partners
  • Maintain accurate, timely, and compliant documentation
  • Manage assigned caseload and ensure continuity of care
