Job Description

BIACON FCSS
is seeking a dedicated Residential Specialist to join our team in providing care and support to individuals in a residential setting. The ideal candidate will have experience in behavior management, nursing home, long term care, and working with people with developmental disabilities. Description Serve as the primary contact. The Program Specialist within the Home and Community-Based Services programs (HCBS Programs) plays an important role in ensuring that participants receive the services and supports that meet their assessed needs based on his/her ISP. The PS works closely with and at the direction of the participant, whenever possible, to assess; identify needs; plan and coordinate services as well as to monitor quality of services. The PS monitors these identified services in order to support the participant in their independent living environment. Education, Experience and Qualifications Bachelor's degree including or supplemented by at least 12-18 college credit hours in sociology, social welfare, psychology, gerontology, or other social Sciences from an accredited college or university. Qualifications Must have a valid PA Driver's License to provide services to participants in their home or community. Personal vehicle must be insured. Shall function as part of a team and work cooperatively with colleagues and supervisor. Well organized and be able to manage own schedule and be flexible to meet the needs of the participants. Excellent verbal and written communication skills Basic computer skills with knowledge of Word, Excel and data entry using Web-based systems. Trustworthy and able to work from remote sites with minimal supervision (if needed). Responsibilities include and are not limited to: Core Functions Assessment
  • Conduct an accurate evaluation of each participant's strengths, needs, preferences, supports and desired outcomes utilizing appropriate assessment tools and input from other professionals, as needed.
Service Plan Development Completes case management narrative annually and when there is a change in the participant's condition. Collaborate with participants, their families and PS Agency utilizing a person-centered planning model to design and modify the ISP; Ensure participant needs, preferences and goals based on information in the ISP and case management narrative are met. Monitoring
  • Ensure that participants receive services authorized in the ISP utilizing service providers of their choice.
The PS is responsible for reviewing and monitoring the plan throughout the plan year with the participant in order to ensure services are still needed. Monitoring is conducted by regular contact with each participant as follows: A minimum of four (4) face to face visits per year which review the participants' need, goals and outcomes. One of these visits will be the annual ISP review. Regular monthly phone calls monitoring the participant's services. Annual plan reviews, quarterly monitoring visits and phone calls shall be completed and documented. More frequent calls or visits shall be made if deemed necessary to ensure the participant's health and safety Remediation
  • Collaborate with the participant and his/her team to resolve issues as well as anticipate issues and work to resolve them in a proactive manner. Report and document Critical Incidents and Complaints in accordance with OLTL regulations Documentation
  • Program Specialist documents all activity conducted on behalf of a participant. Documentation entered in either HCSIS (under 60 waivers) or SAMS (over 60 Waiver) in accordance with established guidelines. The primary means of documentation are Progress/Service Notes (HCSIS) or Journal Notes (SAMS). Incident Reporting
  • Enterprise Incident Management (EIM) is a comprehensive, web-based incident and complaint reporting system that will provide the capability to record and review incidents for Office of Long-Term Living (OLTL) program participants.
Use to Record incidents; Investigate incidents and Track and trend incident data for quality improvement activities Understand, accept and practice the principles of consumer control and independent living.
