A Program Coordinator coordinates ongoing program activity, or a group of related projects, for a company or organization, usually performing administrative or human resources tasks.
TriSource is seeking dedicated Program Specialists/Case Managers to support an IDD non-profit organization located in Pottstown and Collegeville, PA. In this role, you will lead the charge in developing and implementing comprehensive, individualized programs that maximize the medical, intellectual, social, and emotional well-being of the residents. You will collaborate with families and community resources to ensure regulatory compliance.

Responsibilities:
Caseload:
Manage a caseload of 25-30 clients
Lead Individualized Care:
Serve as the team leader in developing, coordinating, and evaluating Client-Centered planning goal strategies
Monitor & Evaluate Plans:
Assess individual program plans to ensure compliance with state and agency regulations, covering written programs, financial/banking plans, medications, and general well-being
Support and Train Staff:
Provide regular training, perform monthly home/individual visits, and mentor direct service staff to resolve operational challenges
Coordinate Transitions:
Manage individual placement, transfer, and discharge planning according to agency and government regulations Act as
Primary Liaison:
Build strong partnerships with families, day programs, and community services to track progress and anticipate needs
Leadership Collaboration:
Actively participate as a member of the management team to solve complex problems and plan for future operational opportunities
Flexible Coverage:
Perform assigned evening, weekend, direct service, and on-call duties
Requirements:
Bachelor's Degree in Human Services (or related field) Case management experience is preferred Experience working with children Valid Driver's License, personal vehicle, and proof of auto insurance Active CPR/First Aid Certification in Medication Administration Ability to list up to 50 lbs Apply today! #IND1
