We are dedicated to assisting eligible nursing facility residents in finding suitable community housing and ensuring smooth, person-centered transitions. Our team is passionate about advocacy and delivering supportive, seamless services. Job Overview We are seeking an energetic and detail-oriented Administrative Program Coordinator to join our dynamic team. In this vital role, you will manage the Transitional and Supplemental Services Coordination for EPCS discharge planning. It includes a variety of administrative functions, support program operations, and ensure efficient communication across departments. Your organizational skills and proactive approach will help us deliver exceptional service to our clients while maintaining smooth office workflows. Key Responsibilities Transitional Services & Supplemental Services Coordination Serve as the primary coordinator for NF Transitional Services and Supplemental Services. Work with clients, supports, case managers, landlords, and vendors to identify approved service needs. Coordinate purchases, deliveries, move-related services, and household setup activities. Schedule and track service completion timelines. Ensure services are delivered according to program requirements and approved budgets. Follow up with clients and supports to confirm services have been completed successfully. Client & Stakeholder Communication Communicate regularly with clients, family members, guardians, nursing facilities, housing providers, and community supports. Administrative Support Maintain tracking spreadsheets and program databases. Prepare reports and documentation as requested. Monitor workflows to ensure services are progressing appropriately. Assist with referrals, enrollments, and program-related administrative tasks. Support office operations, scheduling, document management, and general administrative needs. Perform additional duties as assigned to support program and organizational goals. Provide updates regarding service status and pending needs. Assist in coordinating transition-related activities leading up to discharge. Build positive working relationships with community partners and vendors. Experience Previous office experience with strong computer literacy skills in Microsoft Office Suite (Word, Excel, Outlook) Tracking budgets, purchases, or financial records Proven clerical experience including data entry, filing, proofreading, and managing multi-line phone systems Demonstrated ability to handle confidential information responsibly with excellent organizational skills Strong customer service background with effective phone etiquette and client communication skills Healthcare, social services, case management support, housing services, or administrative coordination.
