Consumer Directed Care Program Care Manager - Hybrid Schedule Seeking a full time (37.5 hours/week) Consumer Directed Care Care Manager. The Consumer Directed Care Care Manager works within the Home Care Department. Responsibilities include conducting home visits; assisting with implementation of Consumer Directed Care service delivery model by assessing and enrolling consumers. Developing, implementing and monitoring a plan of care. Providing ongoing support for home care consumers while meeting home care regulations and requirements. Provide assessment for CDC, support, and annual updates to SCO CDC consumers. Completing required CDC opening packet and working closely with the Fiscal Intermediary to ensure that all forms are complete and accurate to initiate the services. Ability to multi-task and establish priorities.

Qualifications:

Bachelor's degree and Care Management experience preferred. Desire and commitment to working with older individuals required. Excellent written, verbal, interpersonal communication, and organizational skills required. Computer proficiency, including experience with Microsoft products (Excel, SharePoint, Word) and working within a database. Ability to multi-task and establish priorities, in a fast-paced environment. Must have moderate physical mobility; Ability to lift and carry objects weighing up to 20 pounds. Use of a reliable vehicle including active vehicle insurance with a current license to drive required.

• A staged hiring bonus will be awarded in 2 parts, half at 120 Days, and half at 1 year of active employment. The bonus for this position is $1,000.
