Our team is committed to creating meaningful change and delivering impactful programs to help fulfill our mission. We are seeking an experienced ECM Lead Care Manager who will support clients with their health and social needs through the Assessment and creation of care plans, care plan goals, patient and family education and resource referrals, and coordination with medical, social and health plans. Ensure compliance with regulatory and compliance requirements, program effectiveness, and strive to support the organization's growth and impact. Something More offers various programs including:
Enhanced Care Management:
Contracts with all major health plans serving Orange, Los Angeles, Riverside and San Bernardino counties.
Housing Services:
Effectively delivering housing navigation, housing sustaining and housing deposit services in 4 counties.
Day Habilitation:
Day habilitation and training programs
CHW Services:
Community Health Worker services provided as part of the Care Team as well as services in Los Angeles County in partnership with LA Care.
Other Programs:
Mentorship Programs, Life Skills curriculum, Educational Opportunities, CHW Training, and Employment resources as defined by our mission. Position Summary The ECM Lead Care Manager contributes to success of our programs, ensuring that program goals are met and align with the organization's mission. This position is responsible for building relationships with stakeholders and clients, establishing referral resources, ensuring our clients are served with a Trauma Informed approach, dignity and respect. The qualified individual will play a key role in building programs and driving program success, mentoring staff and supporting organizational growth goals.
Enhanced Care Management Program:
Responsible for operationalizing and effectively delivering enhanced care management as defined by the managed care plan contracts, as well as DHCS requirements. Implement new programs, develop care plans, support ICT meetings, and care coordination efforts to ensure appropriate and timely progress occurs. Identify and develop key metrics and reporting to ensure compliance with program requirements. Key Responsibilities Care Coordination support Support the design, planning, and implementation of programs to achieve organizational and program-specific goals. Oversee daily program care coordination responsibilities. Ensure clients are connected to medical and social resources, education, health and mentorship opportunities. Participate with other functional areas to deliver coordinated care.
• Manage a case load of up to 50 clients Provide education, care plan review, attend ICT meetings, support family needs. A high emphasis on San Bernardino Community engagement in person. Develop and update content and tools to standardize program practices and ensure compliance with best practices. Team and Organizational Participation Support the care team, fostering a collaborative, mission-focused environment. Complete continuing education as required. Provide coaching, education and care coordination for ECM staff and their clients enrolled in programs offered by Something More. Foster teamwork, communication, and accountability across program teams to achieve high standards of service and results. Program Evaluation and Reporting Collect data on program activities, participant outcomes, and performance metrics to evaluate program effectiveness. Support preparation of materials and groups. Use lived experience insights to evaluate programs and approaches.
• Support the implementation of technology and tools to ensure compliance with managed care plan requirements, as well as regulatory requirements of all programs. Community and Stakeholder Engagement Cultivate and maintain relationships with community partners, funders, and other stakeholders to support program goals. Represent the organization in community settings, advocating for the mission and building awareness about program offerings. Identify and pursue partnerships that enhance program impact and resource sharing. The Community Health Worker, Care Coordinator will have a passion for serving the underserved. This candidate should have lived experience which reflects that of the populations of served.
Qualifications Education :
High school diploma or equivalent is required. AA or BA preferred. Experience in lieu of higher education will be considered.
Experience :
ECM and Care Management experience is required. CHW training or equivalent experience within the nonprofit sector, including experience supporting teams, developing tools, and educational materials.
Communication :
Excellent interpersonal, public speaking, and written communication skills. Bilingual Spanish preferred.

Experience with leadership roles is highly valued. Ability to lead as well ability to provide reliable leadership support to the Executive Team is essential.

The hourly rate for this position is $23.00- $28.00 depending upon experience.
