Job Description

Purpose of Position The Fatherhood Navigator supports the implementation of the Fatherhood Initiative Program in Indian River County by recruiting and engaging expectant fathers and fathers with young children through evidence-based education, mentoring, and supportive services. This position is responsible for coordinating program activities, ensuring compliance with contractual deliverables and performance outcomes, and supporting ongoing program evaluation to maintain quality and contract compliance. The Fatherhood Navigator works collaboratively with the Fatherhood Coordinator, Director of Programs, and Director of Finance and Operations to ensure the successful implementation, execution, and continuous improvement of the Fatherhood Initiative Program while advancing efforts to reduce racial disparities in maternal and infant health outcomes. Responsibilities include but not limited to the following: Conduct outreach to identify, recruit, and enroll expectant fathers and fathers with young children into educational groups, mentoring, coaching, and other Healthy Start programs and services. Conduct individualized needs assessments and facilitate referrals to community resources that support fathers and their families, including behavioral health, substance use treatment, health care, employment, education, family reunification, and reentry services. Conduct limited biopsychosocial assessments to evaluate participant needs and determine appropriate program enrollment, referrals, and supportive services. Conduct home, community, virtual, or office-based visits with enrolled fathers as assigned. Complete all required documentation, assessments, referrals, and data entry within established program timeframes and standards. Submit accurate and timely documentation to support reimbursement for completed visits. Participate in required trainings, staff meetings, and professional development activities. Document and report on referral accuracy, goals, and completion according to program standards. Ensure training requirements are completed timely and reported to the Fatherhood Coordinator. Maintain all client files in a confidential manner in accordance with HIPAA regulations. Adhere to coalition policies and procedures. Other Duties as assigned. Compensation Structure This is a per diem, as-needed position Compensation is based on successfully completed and documented participant visits. Work schedules may vary based on participant availability and program needs, including occasional evening or weekend visits Qualifications, Training and Experience Bachelor's degree preferred or equivalent work history experience Proficient in Microsoft Office Healthy Start Coalition Structure and system of care Knowledge of community resources. Professional demeanor is required as you will interact with persons at all levels within the organization, outside of the organization and within the community. Bilingual proficiency in English and Spanish (spoken and written) is required Valid Driver's license and vehicle insurance Lead and model professional behavior to staff Work independently, and meet deadlines Work well within a team environment Lift up to 25 lbs. Maintain a positive work environment Flexible work schedule if needed
