You will not be directed step-by-step. You will be given clear goals and the authority to achieve them. If this sounds like the right fit, read on. You will report directly to the Owner/Executive Director. Key Responsibilities Client & Caregiver Relationships (60%) Own client satisfaction from intake through ongoing care Build and maintain trust with clients and their families Serve as primary point of contact for client concerns Conduct care plan reviews and regular client check-ins Manage caregiver-client compatibility and relationships Resolve scheduling conflicts and care disruptions promptly Participate in on-call rotation for after-hours coverage Operations & Referral Development (40%) Support referral relationships with hospitals and discharge planners Conduct new caregiver interviews and onboarding coordination Maintain compliance accuracy on authorizations and care plans Oversee scheduling function and shift coverage protocols Coordinate with onboarding manager on compliance timelines Manage CRM accuracy and client/caregiver documentation Identify and escalate operational issues before they become crises
WHAT THIS ROLE IS NOT
We want you to apply with clear expectations. This role is not remote, it requires physical presence with clients, caregivers, and referral partners. It is not a 9-to-5 with no after-hours responsibility; home care is a 24/7 business and on-call rotation is real. It is not a task-execution role where someone will direct every step. WHAT WE ARE LOOKING FOR
Required 3+ years in home care, healthcare, or high-relationship service Demonstrated ability to own outcomes, not just complete tasks Experience managing client or customer relationships independently Comfortable having difficult conversations with clients and families Strong written and verbal communication skills Proficient in scheduling software and CRM platforms Responds to urgent situations with calm and decisive action Strongly preferred Prior home care agency experience in scheduling, engagement, or compliance Familiarity with home health authorization and care plan processes Experience building referral relationships with healthcare facilities Associate's or bachelor's degree in healthcare, business, or related field Experience supporting or leading a small administrative team
