Job Duties At-A-Glance Participant Case Management Support Services Coordination Administration & Reporting Property Rounds Coordination Scope of Responsibilities:
Participant Case Management Conduct weekly check-ins with residents and participants Maintain regular, secure case notes on each participant's needs, accomplishments, and concerns Conduct intakes, orientations, and participant assessments Assist Reentry Engagement Specialist with participant exit reviews and logging data 2. Support Services Coordination Coordinate referrals and connect participants to outside support services Assist Reentry Engagement Specialist as a liaison between referring organizations and Nehemiah Assist participants with lease signing and documentation, in partnership with the Director of Reentry Program Housing Communicate advocacy opportunities and participant progress data to the Reentry Navigation Coordinator 3. Administration & Reporting Maintain data tracking systems and produce weekly impact reports Compile and submit monthly program reports to the Vice President of Reentry Services for review before they go to senior leadership Compile end-of-year program numbers for accounting and city reporting Schedule staff meetings (the Vice President sets the agenda and leads them) Manage timely submission of timesheets for all Reentry staff Complete grant reporting and invoicing; assist the Vice President with funder relationships and proposal writing as assigned Maintain an accurate database of participant contact information 4. Property Rounds Coordination Schedule property rounds and serve as the primary point person for rounds coverage Partner with Reentry Engagement Specialist for property rounds Coordinate a periodic cross-role check-in on property conditions, resident needs, and rounds coverage Key Responsibilities Coordinate
