Job Description

The Diabetes Program Coordinator oversees the Diabetes Self-Management Education (DSME) Program, ensuring high-quality, patient-centered education and compliance with American Diabetes Association (ADA) recognition standards and regulatory requirements. This position provides program leadership, coordinates quality improvement activities, supports staff development, and provides individualized diabetes education. Essential Duties & Responsibilities Coordinate and oversee all aspects of the DSME program. Develop and monitor the annual program plan and program outcomes. Ensure compliance with ADA recognition standards, regulatory requirements, and hospital policies. Coordinate quarterly DSME Advisory Committee meetings and serve as a liaison between program staff, leadership, and community partners. Participate in quality improvement activities and use program data to improve services. Complete required annual reports and documentation for the ADA and South Dakota Department of Health. Mentor and support diabetes education staff and promote professional development. Provide evidence-based diabetes self-management education and support. Assess patients' clinical, emotional, behavioral, and learning needs and develop individualized education plans. Collaborate with patients, families, caregivers, and the interdisciplinary healthcare team to promote effective diabetes management and improved outcomes. Support coordination between diabetes education and cardiac rehabilitation services. Maintain required licensure, certifications, competencies, and continuing education. Maintain confidentiality and adhere to all hospital policies and procedures. Perform other duties as assigned. Qualifications Current Registered Nurse (RN) license in the State of South Dakota. Minimum of one year of clinic and/or hospital experience preferred.
BLS/ACLS
certification required. Certified Diabetes Care and Education Specialist (CDCES/CDE) preferred, or willingness to obtain within six months of hire. Knowledge of diabetes self-management education and support principles. Strong organizational, communication, leadership, and interpersonal skills. Ability to coordinate program operations and work effectively with an interdisciplinary team. Commitment to ongoing professional development and diabetes education.
