Job Description

Job Summary Assists in the management, development and oversight of the individual support plan. Ensures that the individual support plan is implemented as written and initiates monthly services reviews to record and determine progress. Assists in supervision and oversight of day and residential services. Collaborates with ISC's, Conservators/Family Members, Therapists, and members of the clinical management team by performing the following duties: Relies on knowledge, experience and judgment with limited direction and oversight to accomplish tasks. Sets meaningful, challenging performance objectives with clear measures of success for themselves and team. Effectively organizes and balances tasks and priorities to keep multiple projects on track. Has an authentic interpersonal style that engages others and encourages high performance. Uses personal influence to encourage and support. Shares information. Listens and involves others. Clearly conveys ideas in a manner that engages others and helps them understand and retain the message. Provides timely and professional service to both internal requests and the requests of the people we support; is responsive to the people we support needs and requests. Is always courteous to the people we support and considers the needs of the people we support when making decisions. Understands and follows company policies; encourages others to follow work rules; calls attention to actions that may violate policies and procedures. Complies with basic job requirements and tasks; arrives at work on time as scheduled; proactively informs others when he/she encounters problems that may limit his/her ability to meet expectations. Analyzes and summarizes status of progress by completing monthly service reviews, determining appropriateness of programs and recommending changes as appropriate. Reviews program documentation for accuracy so that billing of services rendered can be submitted for reimbursement. Ensures all aspects of the residential activity, including appearance of home, leisure materials, cleanliness and adequate staffing, documentation, etc. are operating smoothly. Works closely with other clinical/professional staff to maintain communication and provide feedback, standardize procedures and expedite Individual Support Plan (ISP) implementation. Participates in and attends Circle of Support meetings as a representative for the person supported as well as Envision. Reports to the QIP Committee the monthly status of PCP implementation tasks for persons assigned on their caseload. Communicates regularly with the Independent Support Coordinator and other members of the circle of support if amendments to the PCP are needed. Documents all communication. Direct supervisory responsibility for two or more employees. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include recommendations for hiring, and training employees; planning, assigning, and directing work; reviewing performance; rewarding, motivating, and promoting team performance; addressing complaints and resolving problems. At least two years' experience working with persons with developmental disabilities is required. Certain other Human Services Degrees may also qualify.
