The incumbent assumes the responsibility for the day-to-day coordinion of case management, referrals, discharge planning, Homeless Operions Management and Evaluion System (HOMES) monthly compliance review, Homeless GSA fleet rotion, mentoring and training staff. Duties include but are not limited to: Serves as the subject mter expert on the local housing market and collabores with stakeholders within the JHQVAMC and community partner networks, including public housing authorities (PHA), community and government agency executive leadership, and corporion leadership to develop affordable housing options in the Mountain Home cchment area. Provides housing services to Veterans receiving services in theHUD-VASH
program. Provides outreach, crisis intervention, and case management services to homeless and precariously housed Veterans an advanced practice level.
