Assisted Living Transitional Specialist Gemstone Senior Living Communities San Antonio, TX Job Details Part-time | Full-time From $25 an hour 14 hours ago Qualifications Microsoft Office Schedule management Records management Attention to detail Property management Full Job Description Transition Coordinator / Implementor Position Summary We are seeking a self-starter to serve as a Transition Coordinator / Implementor who can independently manage multiple projects with minimal oversight. This position requires someone who is highly organized, detail-oriented, proactive, and an effective communicator. The Transition Coordinator / Implementor will work closely with ownership, the CFO, operations, legal, IT, lenders, vendors, and other members of the acquisition team to ensure properties are properly prepared for transition and that operational improvements are successfully implemented. A significant portion of the position will involve providing regular updates to ownership through Microsoft Teams, documenting progress, identifying outstanding items, and ensuring deadlines are met. The anticipated workweek will be approximately three days focused on Transition Coordinator responsibilities and two days focused on Implementor responsibilities , with flexibility based on current projects and acquisition timelines. Preferred Experience Construction experience, particularly as a construction supervisor or estimator, is preferred. Project management experience is also highly desirable. Experience coordinating multiple vendors, contractors, inspections, and projects is a plus. Experience in senior living, healthcare, real estate, acquisitions, or property operations is beneficial but not required. Transition Coordinator - Pre-Close Responsibilities Once an asset has a signed Purchase and Sale Agreement (PSA), the Transition Coordinator will assume responsibility for coordinating the pre-close transition process.
Responsibilities include:
Initiate the acquisition transition process immediately following execution of the PSA. Conduct and coordinate standard property inspections in accordance with the acquisition checklist and handbook. Identify all capital expenditure (CAPEX) projects and coordinate the process of obtaining bids from qualified contractors and vendors. Attend the due diligence inspection walk with ownership and take detailed notes regarding findings, next steps, additional inspections, repairs, and outstanding items. Gather existing vendor and service contracts and provide them to the CFO for review and analysis. Coordinate any additional reports or documentation requested by the CFO or acquisition team. Maintain and update the acquisition checklist and ensure all supporting documentation is uploaded to the appropriate Microsoft Teams folders. Serve as the primary liaison for lender inspections, appraisals, environmental assessments, and related due diligence activities , while keeping the CFO and lender informed of timelines and outstanding reports. Coordinate with the appropriate parties regarding fire and life safety inspections and requirements . Provide current and professional marketing photographs to Matt Rhode as requested. Coordinate transition town halls , including scheduling and required notices. Track all pre-close action items and proactively communicate delays, concerns, or items requiring ownership decisions. Transition Coordinator - Post-Close Responsibilities Following the closing of an acquisition, the Transition Coordinator will oversee the completion of transition-related items to ensure the property is fully prepared to operate under new ownership.
Responsibilities include:
Ensure all signage is rebranded and that there is no remaining visible presence or branding from the previous ownership. Ensure new uniforms are onsite and available by closing . Work closely with IT to ensure all technology and communication systems are ready for transition, including: Website updates and activation Telephone systems Internet and other necessary technology Ensure the community has adequate food, medical, and other essential supplies available for the transition. Work with our Regional Business Office Manager and the appropriate team members to ensure utilities are transferred and operational without interruption . Confirm all necessary vendors are ready to begin service , including landscaping, vehicles, physical therapy, and other contracted services. Coordinate with legal regarding all necessary post-close requirements , including inspections, licenses, violations, and outstanding regulatory matters. Coordinate and track completion of lender-required repairs . Coordinate and track completion of insurance-required repairs . Maintain a post-close transition checklist and provide ownership with regular updates on outstanding items and completion status. Identify and escalate any issues that could negatively impact the community's operations following closing. Implementor - Operations The Implementor will work closely with the Operations team to take the lead on implementing new ideas, programs, renovations, and improvements across the communities.
Responsibilities include:
Take the lead on implementing new ideas, renovations, programs, and operational improvements identified by ownership or the Operations team. Develop an implementation plan, timeline, budget, and list of required resources for assigned projects. Coordinate with vendors, contractors, community leadership, and internal departments to ensure projects are completed successfully. Track project progress and provide regular updates to ownership through Microsoft Teams. Identify potential obstacles and proactively develop solutions. Ensure completed projects meet the intended goals, quality standards, budget, and timeline. Coordinate projects designed to improve resident experience, engagement, quality of life, and overall community operations . Examples of Implementor Projects Projects may include, but are not limited to: Renovating or improving a movie room Implementing a birthday meal program for residents Implementing a golf simulator Developing new resident activities and engagement programs Implementing chair yoga Coordinating bowling activities Developing new programs designed to improve resident wellbeing Renovating common areas or amenities Implementing other new ideas or improvements identified by ownership and Operations Key Skills & Qualifications The ideal candidate will be: A self-starter who can work independently with minimal supervision Highly organized and detail-oriented Strong in project coordination and follow-through An effective written and verbal communicator Comfortable communicating directly with ownership and senior leadership Comfortable managing multiple projects and deadlines simultaneously Proactive in identifying problems and finding solutions Comfortable coordinating contractors, vendors, inspections, and internal teams Proficient with Microsoft Teams and other standard office technology Able to document progress and maintain accurate project records Comfortable working in a fast-paced acquisition and operations environment Willing to travel to communities and properties as needed Ideal Background The strongest candidates may have experience in: Construction supervision Construction estimating Project management Property management Senior living operations Real estate acquisitions Facilities management Renovation/project coordination Multi-site operations Overall Goal of the
