Must have two years of verifiable experience providing service coordination to people with IDD. This position has seven areas of responsibility: 1) to provide services and supports for individuals transitioning from a Nursing Facility; 2) to provide services and supports for individuals for diversion purposes; 3) to provide enhanced community coordination for people with intellectual and developmental disabilities 4) to monitor and complete all PASRR Alerts/Evaluations for individuals with an intellectual or developmental disability through the TMHP portal; 5) to coordinate and report all of the PASRR training for individuals receiving PASRR and for PASRR Service Coordinators; 6) to complete CARE duties related to PASRR; 7) and to provide service coordination duties on an as needed basis.
