IDD Transition Coordinator Andrews Center - 3.4 Tyler, TX Job Details Full-time 1 day ago Qualifications Experience working with individuals with neurodevelopmental disorders Experience working with individuals with cognitive disabilities Developmental disability care assistance Coordination of services case management method Intellectual disabilities support Social work care coordination Experience working with individuals with intellectual disabilities Disability services experience Full Job Description An Associates degree or Bachelors degree in social, behavioral or human service field is required. Must have two years of verifiable experience providing service coordination to people with IDD. This position has seven areas of responsibility: 1) to provide services and supports for individuals transitioning from a Nursing Facility; 2) to provide services and supports for individuals for diversion purposes; 3) to provide enhanced community coordination for people with intellectual and developmental disabilities 4) to monitor and complete all PASRR Alerts/Evaluations for individuals with an intellectual or developmental disability through the TMHP portal; 5) to coordinate and report all of the PASRR training for individuals receiving PASRR and for PASRR Service Coordinators; 6) to complete CARE duties related to PASRR; 7) and to provide service coordination duties on an as needed basis.
