Job Description

Position Summary Safeway Residential LLC is seeking an organized, dependable, and person-centered QDDP to support our developmental disability services. This position provides clinical and program oversight for a caseload of fewer than 20 individuals and works collaboratively with individuals, families, sponsor providers, direct support professionals, support coordinators, and agency leadership. The QDDP will maintain a consistent weekly office schedule and participate in assigned monthly days for home visits, staff or sponsor training, documentation monitoring, and quality-assurance activities. Some schedule flexibility is necessary for ISP meetings, urgent program needs, and required field visits. Primary Responsibilities Monitor services to ensure they are person-centered, implemented as written, and consistent with each individual's ISP and identified support needs. Review progress notes, data, service documentation, risk-management information, and other required records for completeness, accuracy, timeliness, and quality. Complete or coordinate required assessments, quarterly reviews, reports, plan updates, and documentation by established deadlines. Conduct scheduled and as-needed visits to sponsored residential and supportive in-home service locations. Provide training, coaching, and technical assistance to DSPs and sponsor providers regarding ISP implementation, documentation, rights, risk supports, and person-centered practices. Participate in ISP meetings, care-coordination meetings, discharge or transition planning, and other interdisciplinary meetings as assigned. Communicate concerns, changes in status, incidents, and compliance issues promptly to agency leadership and other authorized team members. Assist with internal audits, corrective actions, quality-improvement activities, licensing readiness, and follow-up monitoring. Maintain confidentiality and comply with HIPAA, Medicaid, DBHDS, HCBS, agency policy, and applicable federal and Virginia requirements. Required Qualifications Meets Virginia requirements to serve as a QDDP, including at least one year of documented experience working directly with individuals who have developmental disabilities. Bachelor's degree in a qualifying human-services field such as social work, psychology, counseling, sociology, special education, or another field permitted by Virginia requirements; or another qualifying professional credential under applicable Virginia regulations. Strong written communication, organization, time-management, and documentation-review skills. Working knowledge of person-centered planning and services for individuals with intellectual or developmental disabilities. Valid driver's license, reliable transportation, and ability to travel to service locations throughout the assigned area. Ability to satisfy applicable criminal history, registry, driving-record, and other employment screening requirements. Preferred Qualifications Experience with Virginia DBHDS-licensed DD services, Medicaid DD Waivers, sponsored residential services, or supportive in-home services. Experience using Therap or a comparable electronic documentation system. Experience training or coaching DSPs and reviewing Medicaid service documentation. Why Join Safeway Residential? Pay:
$24.00 - $27.00 per hour Expected hours: 20.0 - 25.0 per week
