Job Description

The SASH Coordinator/Resident Services Coordinator is responsible for enhancing the quality of life for residents by coordinating supportive services, health and wellness programs, and community resources. This position serves as the primary liaison between residents, healthcare providers, social service agencies, and property management to promote independence, housing stability, and overall well-being. The Coordinator develops partnerships, assesses resident needs, and facilitates programs that support aging in place, self-sufficiency, and community engagement.
Essential Duties and Responsibilities Resident Services:
Conducts needs assessments, creates individualized service plans, connects residents to community benefits, and maintains confidential records.
SASH Coordination:
Collaborates with healthcare partners, leads interdisciplinary care meetings, tracks outcomes, and manages data for grant compliance.
Health & Wellness:
Plans and executes health screenings, fitness classes, nutrition workshops, and educational programming.
Community Engagement:
Builds partnerships with local agencies, fosters an inclusive community environment, and manages volunteers.
Crisis Intervention:
Responds to resident emergencies, provides short-term counseling, and coordinates with emergency services.
Compliance:
Ensures compliance with HUD, Fair Housing, HIPAA, and SASH guidelines while preparing required performance reports.
Property Collaboration:
Partners with property management to identify at-risk residents while strictly maintaining confidentiality.
Qualifications & Competencies Education:
Minimum of a High School diploma or GED.
Experience:
2+ years in case management, social services, healthcare coordination, or affordable housing outreach.
Key Skills:
Knowledge of public assistance programs, crisis intervention, electronic documentation, and strong interpersonal communication.
Physical & Work Environment:
Requires regular interaction with residents and partners, local movement throughout properties for home visits, and occasional lifting up to 25 pounds.

Full-time Compensation:
$19-$21 per hour plus mileage reimbursement.
