Service Coordinators assist individuals and their families in planning, choosing, developing and gaining access to services. They are responsible for identifying individuals needs and coordinating appropriate services. Service Coordinator must be willing to work in a variety of settings. These settings include the business office, individuals' homes, contracted homes, work sites, community activity sites, etc. Service Coordinators are required to travel throughout the state as part of the job. Job Functions and Responsibilities:
- Advocate and provide support to individuals. Work with individuals to promote choice, inclusion and quality of life
- Lead, organize, and facilitate consumer based learning (Life Long Learning) and community based activities
- Assures teams move in agreed upon direction; resolve team conflicts and address barriers through professional leadership.
- Facilitates regular planning meetings for individuals
- Reviews and monitors the service agreement support documentation to assure for accuracy, progress and direction
- Maintains regular contact with families, CMOs, individuals, and other team member and retains documentation of all contacts
- Supervise and support home placements for those individuals who do not live with natural family members
- Supervise and supports Direct Care Professionals who provide the individual with Community/Employment supports
- Maintain familiarity with the individuals daily supports to provide training and to new support staff and/or provide coverage when needed.
- Maintains a valid driver's license and reliable, insured vehicle at all times Must be available for on call to your caseload
Qualifications:
Bachelor's degree in a human services field including but not limited to: sociology, special education, rehabilitation counseling, and psychology OR Associate's degree in human services field with a minimum of 6 years' experience of working directly with people with developmental disabilities. Salary commensurate with experience starting salary $57.000 annually $1000.00 sign on bonus after successful completion of 3 months.
