Position Overview The Part-Time RHP Wellness Coordinator supports AHI's Refugee Health Promotion (RHP) Wellness Program by helping participants access health services, understand available resources, and work toward their health and wellness goals. This position works in partnership with the other RHP Coordinator and reports directly to the Program Director. Each coordinator is responsible for assigned clients, activities, and documentation. Within an 8-12 hour weekly schedule, this role focuses on individual health navigation, Plans of Care, client follow up, and support for wellness activities. Key Responsibilities Health Navigation & Client Support
  • Contact assigned participants to discuss health needs, appointments, and barriers to accessing care.
  • Help participants connect with medical providers, mental health services, and other appropriate community resources.
  • Assist with appointment scheduling and access to transportation and interpretation resources.
  • Follow up on referrals to confirm whether participants received services and identify additional support needed.
  • Provide health resource information within the scope of the position and refer medical questions to qualified healthcare professionals. Plans of Care
  • Work with assigned participants to identify their health needs, goals, and next steps.
  • Explain how a Plan of Care can help participants access services and receive needed support.
  • Gather information through individual conversations, phone calls, and approved online surveys.
  • Complete and update Plans of Care according to program requirements.
  • Track progress and document follow-up services. Documentation, Evaluation & Reporting
  • Enter timely, accurate CaseWorthy notes for services provided.
  • Maintain assigned client records, referral logs, attendance records, and program trackers.
  • Collect surveys and other required program information.
  • Prepare reporting information for assigned clients and activities for review by the Program Director.
  • Protect participant privacy and follow AHI's confidentiality and record keeping procedures. Team Coordination
  • Coordinate with the RHP Coordination team to clarify responsibilities and support consistent client services.
  • Share relevant updates and document handoffs when another staff member needs to follow up.
  • Communicate progress, concerns, and unresolved client needs directly to the Program Director.
  • Take responsibility for completing assigned work within scheduled hours. Qualifications Required
  • At least 1-2 years of experience in public health, social services, education, human services, social work, community development, or a related field.
  • At least 1-2 years of experience in community outreach, health navigation, program coordination, or client support in a nonprofit or community setting.
  • Ability to build trust with immigrant and refugee participants and discuss sensitive needs respectfully.
  • Strong organizational skills and the ability to complete assigned tasks within a limited weekly schedule.
  • Experience with data entry, client documentation, and maintaining accurate records.
  • Excellent English communication skills, including speaking, reading, and writing.
  • Fluency in English and at least one relevant community language, such as Dari, Farsi, or Pashto.
  • Knowledge of Afghan cultural values and experience working with Afghan communities.
  • Ability to work collaboratively while remaining accountable for individual responsibilities.
  • Ability to maintain confidentiality and appropriate professional boundaries.
  • Reliable transportation for assigned in-person responsibilities. Preferred
  • Experience supporting health, wellness, or refugee health programs.
  • Familiarity with local healthcare and community resources.
  • Experience developing individualized service plans or Plans of Care.
  • Familiarity with CaseWorthy, Google Workspace, or similar documentation systems.
