This is a Full Time, 1.00 FTE, Day shift position. A salary range of $55.98 - $83.96 A comprehensive benefits package Relocation Assistance A mission driven healthcare system _________________________________________________________________________________________________________________ Job Summary The Program Manager is responsible for the overall development, implementation, daily operations, and continuous improvement of the Total Joint Replacement Program and the Geriatric Fracture Program. This role serves as the central facilitator between physicians, hospital administration, multidisciplinary clinical teams, rehabilitation partners, and community stakeholders. Will provides patient navigation across the continuum of care—from pre-operative preparation through post-acute follow up (up to 90 days post-surgery). Responsibilities include patient and family education, assessment of clinical and psychosocial needs, coordination of care pathways, data tracking and reporting, program growth strategies, and oversight of standardized evidence-based practices for orthopedic and geriatric fracture populations. Essential Functions Reviews physical and psychosocial assessments and develop individualized plans of care for Total Joint and Geriatric Fracture patients. Monitors patient progress, evaluate interventions, revise plans of care, and support patients and families in coping with hospitalization, surgery, and recovery. Ensures appropriate pre-operative education and preparation, including pre- and post-op teaching classes and distribution of materials to physician offices. Provides follow-up and support for rehabilitation hospitals, SNFs, and assisted living facilities to assess recovery, wound care, therapy progression, and family/social needs (GFP). Serves as a clinical resource for best practices in orthopedic and geriatric fracture care. Coordinates and lead program meetings for both Total Joint Excellence and Geriatric Fracture Program teams, including medical directors, surgeons, unit staff, and administrators. Develops, maintains, and standardizes care pathways, including pre-op, post-op, and discharge order sets for both programs. Maintains effective communication with all team members and departments, fostering strong internal and external partnerships (Rehab, Surgery, Nursing, Pharmacy, Marketing, community physicians, etc.). Leads or participates in community-facing educational initiatives. Tracks and analyzes clinical outcomes, patient satisfaction, complications, LOS, denied days, volumes, and other program metrics. Interprets data to identify trends, develop improvement plans, and report outcomes and recommendations to key stakeholders. Maintain and update program documentation, databases, pathways, and resource materials. Provide education to nurses, physicians, and caregivers on best practices in joint replacement and geriatric fracture care. Participate in orientation and ongoing education for new caregivers involved in program-related care. Advance personal clinical and program leadership skills through continuing education. Co-manage day-to-day program operations, ensuring smooth workflows and safe patient care delivery. Demonstrate financial stewardship and identify resource needs for program sustainability. Chair or co-chair program committees as assigned.
