Job Description
The Outreach & Integrated Referral Specialist (OIRS-II) provides advanced coordination, case management, and cross-program operational leadership to support integrated service delivery across Voices of Tomorrow (VOT) and Zaif programs. This position serves as the primary coordination point for referrals, multidisciplinary collaboration, intensive provider support, and community partnerships while ensuring families and providers are connected to appropriate services. Working independently, OIRS-II manages complex referrals, coordinates multidisciplinary teams, develops community partnerships, and oversees quality improvement activities related to health consultation, provider support, and integrated behavioral health services. The position collaborates closely with clinical staff, program leadership, and external partners to strengthen service delivery, improve family engagement, and advance organizational goals.
Job Responsibilities:
Community Partnership & Network Development:
Develop, cultivate, and maintain strategic referral partnerships with schools, childcare providers, healthcare organizations, community agencies, faith-based organizations, and other community partners. Identify outreach opportunities to expand access to Voices of Tomorrow and Zaif programs. Collaborate with marketing and leadership teams to support community engagement and outreach initiatives. Monitor referral patterns and analyze network data to identify service gaps, partnership opportunities, and community needs. Represent the organization at community meetings, collaborative groups, and networking events as assigned.Cross-Program Referral & Zaif Integration:
Serve as the primary coordination point for referrals across Zaif and Voices of Tomorrow programs. Receive, review, triage, and coordinate referrals from program and clinical staff. Independently determine appropriate program placement based on eligibility, service needs, and program capacity. Coordinate communication between behavioral health, early learning, family support, and other multidisciplinary teams. Facilitate routine cross-program meetings and collaborative case discussions. Respond to complex referral questions requiring knowledge of multiple organizational programs and services. Support cross-program service integration and warm handoffs to improve continuity of care. Monitor child attendance across hub programs and lead intervention planning Track provider compliance and implement strategic action plans to correct flagged concerns Lead evaluation data collection efforts for annual reporting Conduct staff observations and coaching visits to ensure quality family engagement, case management, and service delivery practices. Ensure child files, enrollment and attendance data, health documentation are completed with fidelity Multidisciplinary Team (MDT)Coordination:
Prepare referral summaries, family engagement documentation, and supporting materials for multidisciplinary team meetings. Coordinate MDT scheduling and participation across programs. Monitor participant movement through referral, enrollment, transition, and discharge processes. Ensure required documentation, including consent forms, releases of information, and enrollment records, is complete and current. Track MDT action items and coordinate follow-up activities related to referrals, engagement, and service planning. Facilitate Family Connection Plans (FCPs) and document agreed-upon goals and action steps.Navigation & Service Coordination:
Lead baseline assessment and service navigation for assigned providers and families to ensure timely connection to appropriate services. Supports implementation of Child Care Health Consultation recommendations by reinforcing provider action plans, coordinating follow-up activities, monitoring implementation progress, and connecting providers to additional resources and multidisciplinary supports. Coordinate referrals, warm handoffs, multidisciplinary communication, and follow-up activities across internal programs and community partners. Facilitate communication between providers, families, consultation staff, and partner agencies to support coordinated care. Identify barriers to service engagement and develop strategies to improve access, participation, and successful referral completion. Collaborate with Health Coordinators and other multidisciplinary staff to ensure consultation services are effectively integrated with family navigation and community resource support. Maintain accurate documentation of intake activities, referrals, service coordination, and case progress in accordance with organizational and contractual requirements.Data, Quality Improvement & Evaluation:
Maintain accurate documentation and program records in accordance with organizational policies and contractual requirements. Track referral outcomes, consultation activities, provider engagement, and service utilization. Support data collection, reporting, and evaluation activities for NEST, HEAL, CCHC, and other assigned programs. Analyze program trends and identify opportunities for operational improvements. Participate in quality assurance, compliance monitoring, and continuous quality improvement initiatives. Identify trends and implement continuous quality improvement strategies Support with preparation for monitoring reviews, internal evaluations, funder visits & audits Other duties as assignedQualifications:
BA in human services, public health, nursing, or related fields strongly preferred. 4+ years of progressively responsible experience in program coordination, case management, healthcare, behavioral health, early childhood systems, or related human services. Experience coordinating multidisciplinary teams and managing complex referrals. Experience working with culturally and linguistically diverse communities. Experience supporting quality improvement, compliance, or program evaluation preferred. Experience working with family childcare (FCC) and/or center-based childcare settings, preferred Bilingual /Bicultural (Somali or East African language preferred; Spanish, Chinese, other highly desirable) Prior experience working with families Strong understanding of coordinated service delivery and multidisciplinary collaboration. Excellent organizational, project management, and problem-solving skills. Ability to independently manage multiple priorities and complex workflows. Strong written and verbal communication skills. Experience using electronic documentation systems and Microsoft Office applications. Ability to analyze data and prepare reports to support program decision-making. Knowledge of community resources, referral systems, and family-centered service delivery. Ability to pass a health examination, annual TB test, and criminal background check. Ability to lift 50 lbs. occasionally. Physical dexterity to work with small children, i.e. stooping, kneeling, crouching, reaching, grasping, standing, lifting, walking, climbing, and running quickly. Work will be performed in both inside and outside environments.Pay:
