Hours/Week Compensation:
$42,000 - $50,000 annually, depending on experience, qualifications, and relevant language skills. HOLA is the fiscal agent of the H2N outreach project. Position Summary The Rural Health Transformation Program Support Specialist serves as a key member of the program operations team and plays an essential role in supporting the successful implementation and expansion of Community Health Worker (CHW) initiatives. This position provides coordination, communication, logistics, data management, and operational support to Program Manager, CHW Supervisors, volunteers, and community partners. Through effective organization and collaboration, this role helps strengthen regional CHW infrastructure, community-clinical partnerships, referral systems, workforce development activities, and grant implementation efforts. Specific Job Responsibilities Provide administrative, operational, and coordination support to ensure successful implementation of program goals and grant deliverables. Support the development and sustainability of CHW infrastructure, community-clinical partnerships, and related program initiatives. Coordinate schedules, meetings, timelines, action items, and project activities. Maintain accurate records, databases, documentation, meeting notes, and partner contact information. Serve as a primary communication and coordination resource for staff, volunteers, community partners, and stakeholders. Coordinate volunteer recruitment, onboarding, training, scheduling, and ongoing engagement. Ensure staff and volunteers have the resources, materials, and information needed to support program activities. Support process improvement and strengthen systems that enhance collaboration, communication, and operational efficiency. Prepare outreach materials, training resources, presentations, reports, and meeting packets. Manage office, outreach, and training inventories to support effective program operations. Stay informed about organizational services, community resources, partnerships, and events to support program objectives. Participate in team meetings, trainings, partner meetings, and other organizational activities. Assist with human resources functions, including payroll administration, personnel records, onboarding, and other employee-related processes. Support special projects and perform other duties as assigned. Minimum Requirements Age 18+ Associates degree/Technical Training preferred. Fluency in English (reading, speaking and writing) is required. Hmong, Spanish or Portuguese preferred but not required. Strong interpersonal communication and organizational skills. Professionalism Self-motivated with the ability to work independently with minimal supervision Familiar with Google Suite (Drive, Docs, Sheets), Video Conferencing (Zoom, Microsoft Teams, Webex) Ability to navigate software and Microsoft Suite (Word, Outlook, Excel, PowerPoint) Ability to lift 20 pounds Work effectively and contribute to

Respect, Inclusion, Compassion, Integrity, Community, Cultural Pride and Service Work Schedule and Office Expectations Regular work hours are Monday-Friday, 8 am to 5 pm (1-hour unpaid lunch break). This is an office-based position located at HOLA's Wausau office. The employee is expected to work from the office and be available during regular business hours to support staff, volunteers, community partners, and program operations. Occasional local travel may be required for meetings, trainings, conferences, outreach events, or partner engagement activities.

Compensation The anticipated starting salary for this position is $42,000 annually. Candidates with experience in healthcare, community outreach, case management, program coordination, nonprofit operations, volunteer management, or bilingual/multilingual skills may be considered for compensation higher within the salary range. SIMPLE IRA
retirement plan with voluntary payroll contributions and a HOLA employer match of up to 3% of compensation. Qualified Small Employer Health Reimbursement Arrangement (QSEHRA): Eligible employees may receive reimbursements for individual health insurance and Medicare premiums of up to $300 per month ($3,600 annually), subject to plan terms and eligibility requirements.
