Program Coordinator Essential Job Duties:

Work with the leadership team and core partners to develop, coordinate, and document an annual AHC and CAN action plan that addresses barriers to care, improves the local care system, and addresses social determinates of health. Conduct outreach and organize meetings to engage patients, program participants and partners in the program's mission of eliminating disparities in health outcomes. Recruit and maintain community stakeholders for the All-Babies Initiative (ABI) Advisory Council and subcommittee. Facilitate meetings with community stakeholders to include but not limited to public health officials, clinical and community service providers. Develop presentations and presents information to internal/external program partners and stakeholders in a virtual or in-person formate. Develop a Consumer Leadership Series training for at least 10 active ABI program participants annually. Document all activities, including measurable indicators of success and membership breakdown. Assist with special projects including collecting data and reporting monthly, annual membership surveys, and/or analyzing results. participate in center, project, and partner activities and meetings. Education, Training & Experience Bachelor's degree in nursing, health administration, social work or public health required. Master's degree preferred. Minimum of three years' experience of experience in community organizing and/or performing outreach at a community level preferred. Availability to work a flexible schedule, some evenings and weekends will be required. Proficient with Microsoft Office.
