We do not work weekends or holidays. Our team is supportive, caring, and we have tried to build a work environment that exemplifies teamwork. You must be an LPN with experience in LTC, specifically assisted living, to qualify for this position. Our focus is in assisted living communities. We are offering a full time position to Chronic Care Managers, LPNs, to better serve our clients medical needs. Starting pay is $55k, but with our incentive pay structure (based on your charting and documentation), our CCMs average around $78k/year. A chronic care manager does a combination of a liaison, administrative work as well as case management. Job duties include but are not limited to; order processing, phone call communications with family and other providers, insurance processing, med reconciliation, case management, and maintaining positive relationships with building staff. You are the direct liaison between our company and the facilities we service. You will be working directly with one of our NPs who provides primary care services to our patients. Being a mobile practice we go out to Assisted Living communities to provide services. We go to 1 community each day. Our current need is in both of the Silver Birch communities in Fort Wayne. Due to the unique nature of the position, we do require a 2 hour shadow with one of our training providers. The closest location to Fort Wayne will be in either Muncie or Anderson, IN. This is a way for us to reduce turnover and ensure the position is a good fit for both parties. Training will require travel to Indianapolis for the first week. We will pay for your accommodations. It is a very exciting practice that we have built over the past 10 years. We have successful practices in both Indiana, Illinois and Ohio.
